摘 要:目的观察长皮膏治疗下肢静脉性溃疡的临床疗效,初步探讨其作用机制.方法将60例下肢静脉性溃疡患者随机分为对照组和治疗组.治疗组(30例)使用长皮膏治疗,对照组(30例)使用重组牛碱性成纤维细胞生长因子治疗,两组疗程均为8周.比较两组的疗效、创面愈合率及创面肉芽组织中自噬相关蛋白(LC3-Ⅱ、Beclin-1)的表达.结果对照组总有效率为78.57%,治疗组为86.20%,组间比较有统计学差异(P<0.05);治疗组6周末、8周末时创面愈合率明显高于对照组(P<0.05);治疗组患者肉芽组织中的LC3-Ⅱ、Beclin-1表达水平高于对照组(P<0.05).结论长皮膏治疗下肢静脉性溃疡,能提高创面愈合率,疗效明显优于对照组;可上调创面中LC3-Ⅱ和Beclin-1的表达,从而促进创面愈合.Objective To observe the clinical effects of Ziguizhangpi Ointment (ZG) in the treatment of venous leg ulcer (VLU), and to explore the mechanism. Methods Sixty patients with VLU were randomly divided into control group (n=30) and treatment group (n=30). Patients in control group were treated with bovine basic fibroblast growth factor (bFGF), while patients in treatment group were treated with ZG. After 8 weeks of treatment, the clinical effects, wound healing rate and expression of autophagy related proteins (LC3-Ⅱ, beclin-1) in wound granulation tissue were evaluated. Results The total effective rate of the control group was 78.57%, and that of the treatment group was 86.20%. There were statistically significant differences between the two groups (P<0.05);Wound healing rates of the treatment group 6 and 8 months after treatment were both significantly higher than that of the control group (P<0.05);WB showed higher expression of LC3-Ⅱ, beclin-1 in treatment group than in control group (P<0.05). Conclusion In the treatment of VLU, ZG can improve clinical effects, wound healing rate and up-regulate the expression of LC3-Ⅱ, beclin-1 in wound, so as to promote wound healing.
