全髋关节置换术应用于脊髓灰质炎后遗症期患者的研究进展

摘　　要：脊髓灰质炎后遗症期患者因其下肢肌力不平衡,常出现患侧或健侧髋关节的骨关节炎,严重影响患者生活质量。全髋关节置换术(THA)是治疗髋关节病变的有效方法,但脊髓灰质炎后遗症期患者因其髋关节外展肌无力等问题使其术后脱位风险增加,对患者的预后造成较大的影响,目前国内外此方面的研究较少,因此本文总结全髋关节置换术治疗脊髓灰质炎后遗症期患者的相关文献,从发病机制、手术适应证、手术入路、假体的选择与安放、术后并发症及术后康复训练几方面进行系统综述,以期为全髋关节置换术治疗脊髓灰质炎后遗症期患者提供参考。The patients with poliomyelitis sequelae may have degenerative arthritis of the hip in the paralytic or the nonparalytic contralateral sides due to imbalance of muscle strength,which affect patient’s quality of life seriously.Total hip arthroplasty(THA)is the gold standard treatment for severe refractory coxarthrosis.However,the THA in patients who suffered from poliomyelitis sequelae takes high risk of postoperative complications,such as dislocation resulting from paralysis of muscles around the hip,which impact great deal on the prognosis.Although there are very few studies regarding THA in this patient population,the original articles related to pathogenesis,indications for operation,operative approach,prosthesis choosing,prosthesis implantation,postoperative complication and rehabilitation in recent years were reviewed in this paper to provide reference for clinician who might perform THA for treatment of coxarthrosis secondary to poliomyelitis.
