机器人腹腔镜与普通腹腔镜肾部分切除术治疗肾恶性肿瘤围术期结果比较

摘　　要：目的比较机器人辅助腹腔镜肾部分切除术(RALPN)与普通腹腔镜肾部分切除术(LPN)治疗肾恶性肿瘤的围术期结果。方法收集2017年1月至2018年12月首都医科大学附属北京安贞医院泌尿外科收治的接受肾部分切除术的36例患者的临床资料,根据患者采用的手术方式分为RALPN组(15例)与LPN组(21例)。比较2组患者手术时间、术中出血量、热缺血时间、平均住院时间、术后住院时间和术后并发症发生情况。结果RALPN组患者手术时间、热缺血时间和术后住院时间均明显短于LPN组[(133±28)min比(154±30)min、(23±3)min比(26±4)min、(7.1±0.7)d比(8.3±1.5)d],差异均有统计学意义(均P<0.05)。2组患者术中出血量、平均住院时间比较,差异均无统计学意义(均P>0.05)。2组患者术后均无改为开放手术、术后出血、损伤周围组织、尿外渗等并发症发生。结论与LPN比较,RALPN具有手术时间短、术后恢复快等优势。Objective To compare the perioperative outcomes between robot-assisted laparoscopic partial nephrectomy(RALPN)and traditional laparoscopic partial nephrectomy(LPN)for malignant renal tumor.Methods Clinical data of 36 patients with renal cancer undergoing partial nephrectomy in Beijing Anzhen Hospital,Capital Medical University from January 2017 to December 2018 were collected.The patients were divided into RALPN group(15 cases)and LPN group(21 cases).Operation time,intraoperative blood loss,warm ischemia time,total hospitalization time,postoperative hospitalization time and complications were analyzed.Results Operation time,warm ischemia time and postoperative hospitalization time in RALPN group were significantly shorter than those in LPN group[(133±28)min vs(154±30)min,(23±3)min vs(26±4)min,(7.1±0.7)d vs(8.3±1.5)d](all P<0.05).Intraoperative blood loss and total hospitalization time showed no significant difference between groups(all P>0.05).No complication such as conversion to laparotomy,postoperative hemorrhage,peripheral tissue injury and urinary extravasation occurred in both groups occurred.Conclusion RALPN can shorten operation time and quicken recovery compared to LPN.
