摘 要:目的 在子宫动脉栓塞术中探索多模态影像融合技术在辨认子宫动脉的起源及引导子宫动脉的选择性插管的价值。方法 自2015年11月~2018年12月,23例的女性患者的25支髂内动脉在子宫动脉栓塞术中接受了辅助MIF技术,以辨认子宫动脉起源和引导选择性子宫动脉插管。评定MIF的技术成功率、MIF图像辨认子宫动脉起源的能力;由两位术者术中一致判定MIF图像是否有助于引导子宫动脉的选择性插管。结果 入组的23例患者包括瘢痕妊娠14例、症状性子宫肌瘤7例、症状性子宫腺肌症2例。患者年龄21~44岁,平均年龄为31.9岁。23例患者的25支髂内动脉MIF成像,技术均成功;其中21支(84.0%,21/25)髂内动脉MIF的图像可准确的辨认出子宫动脉的起源;其中15支(71.4%,15/21)髂内动脉的MIF图像被两位术者一致评定为有助于引导子宫动脉的选择性插管。结论 在子宫动脉栓塞术治疗中,髂内动脉的MIF成像有助于辨认子宫动脉起源,并可导引选择性子宫动脉插管。Objective The aim of this study was to assess the utility of multimodality image fusion (MIF) for the identification of the origin of uterine artery and guidance of selective catheterization of uterine artery during uterine artery embolization (UAE). Methods Between november 2015 and december 2018, 23 female patients (25 intern iliac arteries ) received MIF for for identification of the origin of uterine artery and guidance of selective catheterization of uterine artery during uterine artery embolization. The technical success of MIF and the ability of detecting the origin of uterine artery using MIF were assessed. Image fusion was judged whether it was useful to guide the selective catheterization of uterine artery by the two operators in concensus. Results The 23 patients included 14 cases with cesarean scar pregnancy, 7 cases with symptomatic uterine leiomyomata, and 2 cases with symptomatic uterine Adenomyosis. The average age was 31.9 years (21 to 44 years). MIF was technically successful in all the 25 intern iliac arteries of 23 patients. The origin of uterine artery was identified in 21(84.0%, 21/25) of 25 MIF. And 15(71.4%, 15/21) of the 21 MIF were considered useful to guide the selective catheterization of uterine artery. Conclusion The utility of MIF is useful for the identification of the origin of uterine artery and guidance of selective catheterization of uterine artery during UAE.
