摘 要:目的结合Mejer700尿干化学分析仪和IRIS IQ-200尿沉渣分析仪探讨我院检验科全自动尿液分析工作站检测孕产妇尿液镜检的复检规则建立。方法采用全自动尿液分析工作站对我院2018年11月~2019年3月随机选择的1137份孕产妇标本进行检测,同时进行人工显微镜镜检。以镜检结果为标准设置四种复检方案,通过漏检率、复检率、符合率综合评估各规则的合理性及可行性。另选取320份潜在阳性孕产妇尿液标本用于验证复检规则。结果复检方案1、2、3、4的漏检率(假阴性率)分别为8.18%、6.69%、7.74%、1.49%;复检率分别为31.31%、33.51%、22.60%、25.95%;符合率分别为77.92%、78.72%、87.51%、96.66%。采用320份我院孕产妇尿标本对所建立的复检规则(方案4)进行验证,其漏检率为0.94%,复检率为22.18%,符合率为91.60%。结论本研究方案4的复检率较低,漏检率最低、符合率最高,所以首选复检方案4为理想的镜检复检规则。Objective To explore the re-examination rules of maternal urine microscopy by the full-automatic urine analysis workstation in the clinical laboratory of our hospital combined with Mejer700 urine dry chemical analyzer and IRIS IQ-200 urine sediment analyzer. Methods A total of 1137 maternal specimens randomly selected from November 2018 to March 2019 were tested using full-automated urine analysis workstation, meanwhile, manual microscopy was performed. Four re-examination schemes were set up based on the results of microscopic examination. The rationality and feasibility of each rule were comprehensively evaluated based on the missed examination rate, re-examination rate and compliance rate. Another 320 potentially positive maternal urine samples were selected to verify the re-examination rules. Results The missed examination rates(false negative rates) of re-examination schemes 1, 2, 3, 4 were 8.18%, 6.69%, 7.74%, and 1.49%, respectively;the re-examination rates were 31.31%, 33.51%, 22.60%, and 25.95%, respectively;the compliance rates were 77.92%, 78.72%, 87.51%, and 96.66%, respectively. 320 pieces of maternal urine samples were used to verify the established re-examination rules(scheme 4). The missed examination rate was 0.94%, the re-examination rate was 22.18%, and the compliance rate was 91.60%. Conclusion The scheme 4 has the lower re-examination rate and the lowest missed examination rate, the highest compliance rate in this study. Therefore, the re-examination scheme 4 is the preferred ideal microscopic re-examination rule.
