摘 要:目的了解2008-2015年江西省赣州市B型流感病毒株HA与NA基因变异特征。分析WHO推荐的北半球B型流感病毒疫苗株预防效果,为江西省赣州市B型流感监测和防控提供理论依据。方法依据全国流感监测技术指南对获得的18份典型流感样病例鼻咽拭子标本的B型流感病毒进行分离和鉴定。对分离合格的17份样品进行核酸提取,通过逆转录PCR及Sanger测序获取流感病毒HA与NA基因序列信息。采用生物信息学方法,获得HA与NA基因的氨基酸序列,并与WHO推荐的北半球B型流感病毒疫苗株进行比对,绘制毒株相关性热图和进化树。结果与WHO推荐的B型流感病毒疫苗株比较,江西省赣州市17份B型流感病毒在4个HA及8个NA抗原决定簇区域存在多个变异(H>K,N>i等),其中抗原决定簇区域131,137,141,144,163,164,165在江西省赣州市收集的病毒株中发生较大变化(3~7个变异)。结论由此推测,WHO推荐疫苗株对江西省赣州市发现的B型流感病毒株的预防效果可能不太理想,需进一步采集本市B型流感病毒样本并对其变异情况进行监测,寻找针对本市新发B型流感病毒的疫苗株。Objective To understand the genetic variation characteristics of influenza B virus strains HA and NA isolated from Ganzhou city,Jiangxi province from 2008 to 2015 ,and to analyze the preventive effect of the northern hemisphere influenza B virus vaccine strain recommended by WHO on influenza B in Ganzhou city,Jiangxi province,so as to provide theoretical basis for influenza surveillance and prevention and control in Ganzhou city, Jiangxi province. Methods Influenza B virus was isolated and identified from 18 nasopharyngeal swabs of typical influenza-like cases detected in hospitals according to national influenza surveillance technology. Nucleic acid was extracted from 17 samples that met the requirements, and the HA and NA gene sequences of influenza viruses were obtained by reverse transcription PCR and Sanger sequencing. The amino acid sequences of HA and NA genes were obtained by bioinformatics method, and were compared with the northern hemisphere B type influenza virus vaccine strains recommended by WHO,the heat map of the correlation between the strains was drawn and the evolutionary tree was constructed. Results Compared with the strain of influenza B virus recommended by WHO, 17 influenza B virus samples collected in Ganzhou city, Jiangxi province , from 2008 to 2015 showed multiple variations in 4 HA and 8 NA epitope regions( H > K,N > I,etc.),among which some epitope regions( 131,137,141,144,163,164,165 ) showed significant changes in the virus strains collected in Ganzhou city, Jiangxi province ( 3 ~ 7 variations). Conclusion It is speculated that the vaccine strain recommended by WHO may not have an ideal preventive effect on the b - type influenza virus strain found in Ganzhou city, Jiangxi province. It is necessary to collect more b - type influenza virus samples in this city and monitor its variation for further researches, so as to search for the vaccine strain targeting at the newly emerging b - type influenza virus in this city.
