摘 要:目的检测巨噬细胞移动抑制因子(macrophage migration inhibitory factor,MIF)在慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者肺组织中的表达水平及临床意义。方法选取深圳市龙岗中心医院2015年1月至2018年9月因肺大疱行肺大疱楔形切除术患者的肺组织标本为研究对象(n=180),依据病史资料分为COPD病例组(COPD合并肺大疱行肺大疱楔形切除术患者,n=90)和对照组(未合并COPD单纯肺大疱行肺大疱楔形切除术患者,n=90);病例组再根据术前肺功能指标分为:COPD轻度组(FEV1/FVC<70%、FEV1/Pred%≥80%,n=30)、COPD中度组(FEV1/FVC<70%、50%≤FEV1/Pred%<80%,n=30)和COPD重度组(FEV1/FVC<70%、30%≤FEV1/Pred%<50%,n=30)。利用RT-PCR、ELISA和Western blot检测各组人群肺组织中MIF mRNA及蛋白的表达水平,并分析各指标与肺功能的相关性。采用SPSS 18.0进行单因素方差分析、SNK-q检验和Pearson相关分析,以P<0.05为差异有统计学意义。结果与对照组相比,病例组MIF的表达水平显著升高,差异有统计学意义(P<0.05);依据COPD严重程度分组,疾病严重程度越高,MIF mRNA及蛋白表达越强,差异有统计学意义(P<0.05)。病例组MIF表达水平与肺功能指标(FEV1/ FVC%、FEV1/Pred%)呈负相关(P<0.05)。结论COPD肺组织中MIF的表达水平显著升高,和疾病严重程度及肺功能指标明显相关。Objective To investigate the expression of macrophage migration inhibitory factor (MIF) in pulmonary tissues from patients with chronic obstructive pulmonary disease (COPD) and the relationship with its clinical features. Methods One hundred and eighty patients who underwent pulmonary bullectomy lobectomy due to pneumatocele from January 2015 to September 2018 in Longgang Central Hospital were enrolled and classified into patients without COPD (control group)and patients with COPD (COPD group), with 90 patients each group. According to the lung function parameters, 90 patients with COPD were divided into the mild COPD group, the moderate COPD group, and the severe COPD group. The levels of mRNA and protein of MIF were measured with RT-PCR, ELISA and Western blot. One-way ANOVA, Pearson correlation analysis and SNK-q test were used to analyze the results with SPSS 18.0, and P<0.05 was considered statistically significant. Results The level of MIF in pulmonary tissues from the control group was obviously lower than those in the COPD group (P<0.05). The level of MIF in pulmonary tissues in the severe COPD group was obviously higher than those in pulmonary tissues in the mild COPD, moderate COPD and control groups (P<0.05). MIF was positively correlated with the lung function parameters (P<0.05). Conclusion The high expression of MIF in pulmonary tissues is closely related to the severity of COPD.
