摘 要:目的探讨双期CT扫描对下肢静脉血管成像精确度的影响。方法回顾性分析56例怀疑下肢静脉疾病患者,行下肢静脉血管CT扫描,第1期扫描获得图像为单期组,结合第1期和第2期扫描获得图像为双期扫描组。采用配对样本t检验,对比分析2种扫描方式的辐射剂量(容积CT剂量指数、剂量长度乘积及有效剂量)、深静脉血管(腘静脉、股静脉和髂总静脉)图像质量。结果与单期扫描组相比,双期扫描组辐射剂量指标均显著增加(容积CT剂量指数t=-14.33,P=0.04;剂量长度乘积t=-292.2,P<0.00;有效剂量t=-11.86,P<0.00),深静脉血管图像显著提高(腘静脉t=-2.71,P=0.04;股静脉t=-2.71,P=0.04;髂总静脉t=-3.79,P=0.01)。结论双期CT扫描下肢静脉血管成像技术虽然增加辐射剂量,但可提供更准确、更丰富的影像信息。Objective To investigate the effect of dual-phase CT scan on the diagnostic accuracy of lower extremity venous angiography. Methods A retrospective analysis of 56 patients with suspected lower extremity venous disease was performed and the CT scans of the lower extremity venous vessels were collected. The first phase scans were classified as the single-phase scan group, and the combinations of the first phase and the second phase scans were classified as the dual-phase scan group. The image quality of deep veins ( popliteal vein, femoral vein and common iliac vein) and the radiation doses (volume CT dose index, dose length product and effective dose) of these two groups were compared by using paired t test.Results Compared with the single-phase scan group, the radiation dose index of the dual-phase scan group was significantly increased (volume CT dose index t =-14.33,P=0.04;dose length product t=-292.2,P<0.00;effective dose t =- 11.86 , P<0.00), the quality of venous image was significantly improved (popliteal vein t =- 2.71, P =0.04;femoral vein t=-2.71, P = 0.04;common iliac vein t =-3.79, P =0.01). Conclusion Dual-phase CT scanning of lower extremity venous angiography can provide more accurate and enriched image information, although it increases the radiation dose.
