CT增强纹理分析鉴别肾嗜酸性细胞腺瘤和肾透明细胞癌的价值

摘　　要：目的探讨CT增强图像纹理分析技术在肾嗜酸性细胞腺瘤(RO)和肾透明细胞癌(ccRCC)中的诊断及鉴别诊断价值。方法回顾性分析经手术病理证实的11例RO和46例ccRCC患者,所有患者术前均进行CT增强检查,利用Omni-Kinetics纹理分析软件分析2组肿瘤的纹理特征,包括平均灰度值、标准差、偏度、峰度、能量及熵。采用独立样本t检验比较2组计量资料之间的差异,构建ROC曲线评价其诊断效能。结果在CT增强图像上,RO的标准差和熵小于ccRCC,差异均有统计学意义,能量大于ccRCC,差异有统计学意义;平均灰度值、偏度小于ccRCC,峰度大于ccRCC,差异均无统计学意义。在2组肿瘤的鉴别中,标准差AUC为0.875,以25.030为阈值,敏感性为80.77%,特异性为87.50%;能量AUC为0.946,以0.013为阈值,敏感性为92.59%,特异性为88.89%;熵AUC为0.880,以6.593为阈值,敏感性为81.48%,特异性为88.89%。结论 RO和ccRCC之间的纹理特征存在显著差异,标准差、能量及熵对两者之间的鉴别具有重要价值。Objective To assess the value of CT-enhanced image texture analysis in differential diagnosis between renal oncocytoma (RO) and clear cell renal cell carcinoma (ccRCC).Methods 11 cases with RO and 46 cases with ccRCC confirmed by pathology were analyzed retrospectively. All patients underwent CT enhancement before surgery.The Omni-Kinetics texture analysis software was used to analyze the texture features of the tumors, including mean gray value, standard deviation, skewness, kurtosis, energy and entropy.The independent sample t-test was used to compare the differences between the two groups,and ROC curve was constructed to evaluate the diagnostic efficacy.Results Compared to ccRCC,the standard deviation and entropy of RO were significantly lower,and the energy was significantly greater.The mean gray value and skewness of RO were lower than those of ccRCC,and the kurtosis was greater than that of ccRCC, there were no significant differences.For differential diagnosis between the two groups,the area under the standard deviation curve was 0.875;with a threshold of 25.030,the sensitivity was 80.77% and the specificity was 87.50%.The area under the energy curve was 0.946;with a threshold of 0.013,the sensitivity was 92.59 % and the specificity was 88.89%.The area under the entropy curve was 0.880;with 6.593 as the threshold,the sensitivity was 81.48% and the specificity was 88.89%.Conclusion There are significant differences in texture characteristics between RO and ccRCC.Standard deviation, energy and entropy show great value for the differential diagnosis.
