摘 要:目的探讨拉伸指数、单指数模型DWI定量评价慢性乙肝炎症活动度的价值。方法对82例慢性乙肝患者和20例健康志愿者行肝脏多b值DWI检查。通过拉伸指数模型获取参数扩散分布指数(DDC)R512.62、扩散异质性指数(α),通过单指数模型获取ADC。采用Spearman相关分析评价各参数与肝炎症活动度分级的相关性。运用ROC曲线评价各参数对慢性乙肝炎症活动度的诊断效能。结果肝脏的DDC、ADC值均与炎症活动度分级呈负相关(r=-0.719和-0.661,P均<0.001),α值与炎症活动度分级无相关性(P>0.05)。DDC、ADC值诊断炎症活动度≥A2的AUC分别为0.891和0.845,诊断炎症活动度≥A3的AUC分别为0.850和0.845。结论拉伸指数模型参数DDC、单指数模型参数ADC均可用于定量评价慢性乙肝炎症活动度分级,并与炎症活动度分级明显相关。Objective To explore the value of stretched exponential and monoexponential model DWI in quantitatively evaluating inflammation activity in chronic hepatitis B(CHB).Methods 82 patients with CHB and 20 healthy volunteers had undergone multi-b values DWI.DDC and a were calculated by using the stretched exponential model. ADC was calculated by using the monoexponential model.Spearman rank correlation was used to assess the correlation of all the parameters with the inflammation activity grades.ROC analysis was used to evaluate the diagnostic performance of various diffusion parameters in predicting the inflammation activity grade. Results The DDC and ADC showed a negative correlation with the inflammation activity grades (r=-0.719 and - 0.661,all P<0.01).while a value showed no correlation with the inflammation activity grade(P>0.05).The AUC of DDC and ADC values to detect inflammation activity grade ≥A2 were 0.891 and 0.845.The AUC of DDC and ADC values to detect inflammation activity grade ≥A3 were 0.850 and 0.845.Conclusion The DDC from stretched exponential and ADC from monoexponential model value could be used for quantitatively evaluating the grade of inflammation activity in CHB and showed significant correlation with the inflammation activity grades.
