摘 要:目的对黄山市2014~2018年非结防机构疑似肺结核患者报告转诊追踪情况进行分析,查找疑似肺结核患者追踪转诊工作的薄弱环节,采取针对措施,提高结核病患者发现水平。方法收集2014~2018年黄山市结核病信息管理系统中非结防机构报告疑似肺结核患者的转诊、追踪、到位、诊断等资料,对五年间的转诊到位率、追踪到位率、总体到位率、到位患者活动性肺结核检出率、到位患者活动性肺结核排除率、非结防机构对肺结核及涂阳肺结核的贡献率进行统计分析。结果 2014~2018年黄山市结核病转诊到位率总体上呈下降趋势,由40.13%下降至29.39%;追踪到位率总体呈上升趋势,由76.94%上升至92.39%;总体到位率总体呈上升趋势,由86.59%上升至95.47%。到位患者活动性肺结核的检出率呈下降趋势,由55.36%下降至49.85%;排除率呈上升趋势,由38.92%上升至45.21%。非结防机构发现活动性肺结核及涂阳肺结核的贡献率呈相对稳定水平,五年间平均水平仍高于60%。结论应继续加强医防合作,通过加大公众结核病知识宣传力度及临床医生培训力度,提升网络直报信息系统工作质量,提高结核病患者发现水平。Objective To analyze the follow-up referral the situation of referral tracking of suspected tuberculosis(TB) patients reported by non-tuberculosis prevention and control institutions in Huangshan City from 2014 to 2018, find out the weak links in the follow-up referral work of suspected tuberculosis patients, and take corresponding measures to improve the discovery detection level of tuberculosis patients.Methods Data on referral, tracing, arrival and diagnosis of suspected tuberculosis patients reported by non-tuberculosis prevention institutions in Huangshan TB Information Management System from 2014 to 2018 were collected.The referral arrival rate, tracing arrival rate, overall arrival rate, detection rate of arrived patients with active pulmonary tuberculosisactive pulmonary tuberculosis of arrived patients, exclusion rate of arrived patients with active pulmonary tuberculosisactive pulmonary tuberculosis of arrived patients, and contribution rate of non-tuberculosis prevention agencies institutions to for tuberculosis and smear-positive tuberculosis in five years were statistically analyzed.The contribution rate was analyzed.Results From 2014 to 2018, the referral arrival rate in Huangshan City decreased from 40.13% to 29.39%;The tracking arrival rate increased from 76.94% to 92.39%;The overall arrival rate increased from 86.59% to 95.47%. The detection rate of active pulmonary tuberculosis in place decreased from 55.36% to 49.85%;The exclusion rate increased from 38.92% to 45.21%. The contribution rate of active pulmonary tuberculosis and smear-positive pulmonary tuberculosis found in non-TB prevention institutions was relatively stable, and the average level was still higher than 60% in five years.Conclusion Medical and preventive cooperation should continue to be strengthened.
