摘　　要：目的通过分析某患者的病历,探讨患者治疗过程中用药的合理性。方法通过文献检索、病历查阅、治疗全过程跟踪等方法,分析患者治病过程中中药处方的合理性。结果患者诊断为中风病痰瘀互结证,治疗过程以涤痰汤为基本方,根据患者疾病进展的不同阶段和临床表现进行加减,取得良好的治疗效果。结论本例患者临床治疗方证相符,临床用药合理。中药临床药师全过程参与患者中医中药治疗解决方案,确保中药临床药师在临床合理用药中发挥应有的作用。OBJECTIVE To explore the rationality of medication in the treatment of patients by analyzing the medical records of a patient. METHODS The rationality of TCM prescriptions in the course of treatment was analyzed by means of literature review,medical records review and treatment process tracking. RESULTS The patient was diagnosed as apoplexy phlegm and blood stasis syndrome,and the treatment process was based on Ditan decoction,which was added or subtracted according to the different stages of disease progression and clinical manifestations. CONCLUSION The clinical treatment of this case is consistent with the clinical prescription. Chinese medicine clinical pharmacists participate in the whole treatment process could ensure the rational use of drugs.
