摘 要:目的观察红外线治疗对下肢骨折患者的愈合情况及对微循环的影响。方法选取解放军总医院第八医学中心2017年10月~2018年8月期间收治的下肢骨折患者60例,按随机数表法分为对照组和治疗组,各30例。其中对照组给予常规治疗,治疗组在常规治疗基础上使用红外线照射治疗,比较两组患者治疗后的骨折愈合情况和微循环指标变化情况。结果治疗组在治疗后21和28d时测量骨痂密度分别为(1.98±0.22)、(2.87±0.31),明显高于对照组的(1.63±0.26)、(2.05±0.27),差异有统计学意义(P<0.05);足甲襞微循环指标检测结果显示,治疗组治疗5d后的流态积分(0.81±0.13)、管襻积分(0.41±0.06)和襻周积分(0.24±0.08)明显小于对照组,差异有统计学意义(P<0.05);襻顶血管直径(14.69±1.36)、管襻畸形率(20.13±1.57)和白微栓率(1.19±0.18)的结果明显优于对照组,差异有统计学意义(P<0.05)。两组在治疗期间均无不良反应发生。结论使用红外线照射能促进骨折愈合,提高骨折部位微循环,属于物理疗法,安全有效,值得临床推广应用。Objective To observe the effect of infrared therapy on the healing and microcirculation of patients with lower limb fracture.Methods From October 2017 to August 2018,60 patients with lower extremity fracture were randomly divided into two groups:control group (n = 30) and treatment group (n = 30).The patients in the control group were treated with conventional therapy and the patients in the treatment group were treated with infrared radiation on the basis of routine treatment. The changes of fracture healing and microcirculation indexes were compared between the two groups after treatment.Results After treatment,the scab density measured at 21 days and 28 days in the treatment group was 1.98 ± 0.22,2.87 ± 0.31 was significantly higher than that in the control group (1.63 ± 0.26,2.05 ± 0.27,P < 0.05).The results of foot nailfold microcirculatory index showed that the flow pattern score (0.81 ± 0.13),tube loop score (0.41 ± 0.06) and loop circumference score (0.24 ± 0.08) in the treatment group after 5 days of treatment were significantly lower than those in the control group (P < 0.05).At the same time,the diameter of apical vessels in the treatment group (14.69 ± 1.36),the malformation rate of the loop (20.13 ± 1.57) and the rate of white microthrombus (1.19 ± 0.18) were significantly better than those in the control group (P < 0.05). There were no adverse reactions in both groups during treatment.Conclusion Infrared irradiation can promote fracture healing and improve microcirculation of fracture site. It belongs to physical therapy and is safe and effective. It is worth popularizing and applying in clinic.
