四二五合方对早发性卵巢功能不全的疗效观察

摘　　要：目的观察四二五合方对肾虚血瘀证早发性卵巢功能不全患者AMH、FSH、LH、E2 影响,探讨四二五合方对提高女性卵巢功能的依据。方法采取病例对照研究法,选取2017 年1 月—2018 年10 月到本院妇科门诊就诊的中医辨证为肾虚血瘀证的早发性卵巢功能不全患者100例,随机分为实验组及对照组,各50 例,实验组给予中药四二五合方加减治疗,对照组予以戊酸雌二醇联合黄体酮周期治疗,比较两组患者中医症侯积分及血AMH、FSH、LH、E2 的变化判断四二五合方治疗早发性卵巢功能不全(肾虚血瘀证)的临床疗效,连续治疗3 个月,并随访3 个月经周期,统计两组疗效并分析,同时观察血、尿常规及肝肾功检测药物安全性。结果经过3 个月的连续治疗后,中医症侯积分实验组明显下降,对照组无明显变化,两组比较,差异有统计学意义(P < 0.05);实验组患者血清 FSH、LH 较前明显降低,E2、AMH较前升高,对照组患者血清FSH、LH 较前略降低,E2、AMH 无明显改善,实验组与对照组比较差异具有统计学意义(P < 0.05)。治疗过程中患者未出现不良反应及药物毒副作用。结论四二五合方具有温肾活血功效,可影响肾虚血瘀证早发性卵巢功能不全患者AMH、FSH、LH、E2,有效改善卵巢功能。Objective To observe the effect of Si-Er-Wu-He Recipe on AMH, FSH and LH and E2 in patients with premature ovarian insufficiency of kidney deficiency and blood stasis syndrome, Provide clinical basis for improving female ovarian function with Si-Er-Wu-He Recipe. Methods 100 cases of premature ovarian insufficiency with TCM syndrome of kidney deficiency and blood stasis syndrome were selected from January 2017 to October 2018, They were divided into experimental and control group in each group with 50 cases, The experimental group was treated by adding or subtracting the traditional Chinese medicine Si-Er-Wu-He Recipe, while the control group was treated by estradiol valerate combined with progesterone cycle, Comparing the TCM symptom score and the changes of AMH, FSH, LH and E2 in the two groups to judge the clinical efficacy of Si-Er-Wu-He Recipe in the treatment of premature ovarian insufficiency (kidney deficiency and blood stasis syndrome), continuous treatment for 3 months and follow-up for 3 months, Statistical analysis and analysis of the two groups, the efficacy and analysis of the two groups were statistically analyzed, and the safety of blood, urine routine and liver and kidney function tests were observed. Results After 3 months of continuous treatment, the scores of TCM symptoms in the experimental group decreased significantly, and there was no significant change in the control group, the difference was statistically significant between the two groups (P<0.05);the serum FSH and LH of the experimental group were significantly lower than before, and E2 and AMH were higher than before, In the control group, serum FSH and LH were slightly lower than before, and E2 and AMH were not significantly improved, the difference was significant between the two groups (P<0.05). The patient did not experience adverse reactions and drug side effects during the treatment.
