摘 要:目的探讨血必净对爆炸致兔急性肺损伤(ALI)的早期保护作用及其机制。方法将40只家兔随机分为血必净低剂量(L)组、高剂量(H)组、地塞米松(D)组、氯化钠溶液对照(N)组及ALI模型(A)组,每组8只。除了N组外其他组均建立ALI模型。L、H组分别给予血必净10、20ml/kg;D组给予0.5ml/kg地塞米松;N、A组给予10ml/kg氯化钠注射液;各组于给药后24h采集血液及肺组织标本。采用ELISA法测定血清中TNF-α、IL-1β、IL-6、IL-10的含量,采用Western blot法和RT-PCR法测定肺组织核转录因子-κB(NF-κB)、基质金属蛋白酶-9(MMP-9)的蛋白及m RNA表达水平,测定肺湿干重比(W/D),并在光镜下观察肺组织病理变化。结果A组MMP-9、NF-κB的表达水平、TNF-α、IL-1β、IL-6、IL-10含量以及W/D均比N组高(均P<0.05);经血必净治疗后,L、H组MMP-9、NF-κB的表达水平、TNF-α、IL-1β、IL-6含量以及W/D均比A组低(均P<0.05),IL-10表达量比A组高(P<0.05),其中均以H组改变最显著(均P<0.05);肺病理切片显示血必净治疗后肺组织损伤明显减轻。结论血必净可抑制爆炸致ALI早期炎性介质的生成和释放,减轻组织病理损害,从而起到保护作用。Objective To investigate the effect and mechanism of Xuebijing(XBJ) on the acute lung injury(ALI) induced by explosion in rabbits. Methods Forty rabbits were randomly divided into 5 groups with 8 animals in each group. The acute lung injury was induced by explosion;20 ml/kg Xuebijing(group H), 10 ml/kg Xuebijing(group L), 0.5 mg/kg dexamethasone(group D) and normal saline(group N) were given by intravenous injection, respectively;no treatment was given for rabbits in group A. Blood and lung tissue samples were obtained 24 h after model induced. The expression of tumor necrosis factor(TNF)-α, interleukin(IL)-1β, IL-6 and IL-10 in serum was determined by ELISA. The expression of NF-κB and MMP-9 in lung tissue was determined by Western blot and RT-PCR. Also the lung W/D was measured, and pulmonary pathological tissue scores were examined by HE stain. Results The expressions of MMP-9, NF-κB,TNF-α, IL-1β,IL-6,IL-10, and W/D in group A were higher than those in group N(all P<0.05). The expression of MMP-9, NF-κB, TNF-α,IL-1β, IL-6, and W/D in groups H and L were lower than those in group A(all P<0.05);and the expression of IL-10 was higher than that in group A(P<0.05), and the changes were most markedly in group H. Compared to groups A and N, the degree of lung tissue injury was significantly attenuated in groups H, L and D. Conclusion Xuebijing has good protective effects in rabbits with ALI, which may be associated with its inhibitive effect on inflammation.
