摘  要:目的:探讨针刺对关节炎模型大鼠炎性因子、抗炎免疫及抗炎镇痛反应的影响。方法:将30只大鼠随机分为正常组、模型组及针刺组,每组10只,以弗氏完全佐剂注入模型组和针刺组大鼠右后足跖底部制作关节炎模型,针刺组大鼠给予足三里、阳陵泉和昆仑穴3处针刺治疗,正常组和模型组不做任何处理。HE染色观察大鼠关节滑膜组织病理变化;醋酸扭体法、热板致痛法观察针灸镇痛作用;取大鼠脾脏、胸腺计算免疫器官系数;采血后采用还原酶法测定NO含量、羟胺法测定SOD活力、硫代巴比妥酸法测定MDA含量;ELISA检测大鼠足炎症组织IL-1、TNF-α含量。结果:HE染色结果显示针刺缓解大鼠滑膜组织病理变化;针刺组大鼠抗醋酸扭体反应及热痛域较模型组显著提高(P<0.05);胸腺系数较模型组升高而脾系数较模型组显著恢复(P<0.05);针刺组大鼠血清NO、MDA含量均较模型组降低,而体内SOD含量较模型组显著升高(P<0.05);ELISA结果显示针刺组大鼠肌肉组织IL-1、TNF-α含量均较模型组显著降低(P<0.05)。结论:针刺抑制关节炎大鼠炎性因子表达、促进抗炎免疫及抗炎镇痛的发生。Objective: To investigate the effect of acupuncture on inflammatory factors, anti-inflammatory immunity and analgesic reactions in model rats with arthritis. Methods: 30 rats were randomized into the normal group, the model group and the acupuncture group, with 10 rats in each group. Arthritis model was established by intracutaneous injection of complete Freund’s adjuvant into the right hind foot pad of the rats. Points of Zusanli(ST36), Yanglingquan(GB34) and Kunlun(BL60) were selected in the acupuncture group. The other two groups were not given any intervention. HE staining was used to observe the pathological changes of synovial tissues. Acetic acid writhing and the hot plank assays were used to observe the analgesia of acupuncture. Calculating the coefficient of immune organs. Using the methods of nitrate reductase for NO content determination, hydroxylamine for SOD determination and thiobarbituric acid for MDA determination. ELISA was used to detect the changes of IL-1 and TNF-α. Results: HE staining showed that acupuncture relieved the pathological changes of synovial tissue in rats. After acupuncture treatment, the threshold of acetic acid writhing and thermal withdrawal latency were significantly increased compared to those of the model group( P <0.05). The thymus coefficient of the acupuncture group was significantly higher than that of the model group, whereas the spleen coefficient was obviously recovered in the acupuncture group than that in model group( P <0.05). The serum contents of NO and MDA were significant lower in the acupuncture group than those in the model group, and the SOD content was obviously increased in the acupuncture group rats( P <0.05). After the treatment, the contents of IL-1 and TNF-α were significant decreased in the acupuncture group than those in the model group( P <0.05). Conclusion: Acupuncture can promote anti-inflammatory immunity and analgesia by reducing the inflammatory factors in arthritis rats.
