预防性压疮护理在下肢深静脉血栓介入置管溶栓治疗中的应用

摘　　要：目的研究预防性压疮护理在下肢深静脉血栓介入置管溶栓治疗中的应用效果。方法方便选取2017年7-12月期间该院介入科(省级三甲医院)收治的下肢深静脉血栓介入置管溶栓治疗患者25例设为对照组(常规护理),将2018年1-6月期间收治的下肢深静脉血栓介入置管溶栓治疗患者25例为观察组(预防性压疮护理)。将两组的压疮发生率、患肢恢复情况、下床活动时间、住院时间进行比对。结果观察组(0.00%)和对照组下肢深静脉血栓患者的压疮发生率对比差异无统计学意义(χ^2=3.192,P=0.074);观察组患者的患肢恢复时间[肿胀肢体弹力恢复时间(34.21±5.03)h、患肢腿围缩小时间(8.11±2.06)d、肿胀消失时间(24.25±4.70)h]短于对照组(t=6.017、6.650、4.882,P=0.001、0.001、0.001),下床活动时间(4.03±0.45)d、住院时间(15.86±3.62)d均短于对照组(t=16.643、9.019,P=0.001、0.001)。结论对下肢深静脉血栓患者介入置管溶栓治疗中实施预防性压疮护理可减少压疮的发生,有助于预后效果的改善,效果令人满意。Objective To study the effect of preventive pressure ulcer nursing in the treatment of deep venous thrombosis in the treatment of thrombolysis.Methods Twenty-five patients with deep venous thrombosis treated with deep venous thrombosis in our hospital from July to December 2017 were convenient enrolled in the control group (conventional care),from January to June 2018,Twenty-five patients who underwent thrombolysis for deep venous thrombosis were enrolled in the observation group (preventive pressure ulcer care).The incidence of pressure ulcers,recovery of limbs,time of getting out of bed,and length of hospital stay were compared between the two groups.Results There was no significant difference in the incidence of pressure ulcers between the observation group (0.00%) and the control group with deep venous thrombosis (χ^2=3.192,P=0.074).The recovery time of the affected limb in the observation group [swelling limb recovery time(34.21±5.03)h,the shortening time of the limbs (8.11±2.06)d,the swelling disappearance time (24.25±4.70)h] was shorter than the control group(t=6.017,6.650,4.882,P=0.001,0.001,0.001).The time to get out of bed (4.03±0.45)d and the length of hospital stay (15.86±3.62)d were shorter than those in the control group (t=16.643,9.019,P=0.001,0.001).Conclusion The implementation of preventive pressure ulcer care in patients with deep venous thrombosis of lower extremity can reduce the occurrence of pressure sores and improve the prognosis.The effect is satisfactory.
