COOK球囊与前列腺素E2栓在初产妇促宫颈成熟引产中的应用效果

摘　　要：目的探究COOK球囊与前列腺素E2栓在初产妇促宫颈成熟引产中的应用效果。方法选取2016年1月~2018年6月我院接诊的80例初产妇作为研究对象,采用抽签法将其分为实验组和对照组,每组各40例。实验组产妇采用COOK球囊促宫颈成熟引产,对照组产妇采用前列腺素E2栓促宫颈成熟引产。比较两组产妇促宫颈成熟前后的Bishop评分、总产程、产后出血量、分娩方式、并发症总发生率。结果两组产妇促宫颈成熟前的Bishop评分比较,差异无统计学意义(P>0.05);两组产妇促宫颈成熟后的Bishop评分均显著高于促宫颈成熟前,差异有统计学意义(P<0.05);两组产妇促宫颈成熟后的Bishop评分比较,差异无统计学意义(P>0.05)。实验组产妇的总产程短于对照组,产后出血量少于对照组,差异有统计学意义(P<0.05)。实验组产妇的自然分娩率高于对照组,差异有统计学意义(P<0.05)。实验组产妇的并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论COOK球囊与前列腺素E2栓均具有良好的促宫颈成熟的效果,但是COOK球囊促宫颈成熟的安全性更高,可减少新生儿窒息等并发症的发生,改善分娩结局,值得在临床中广泛推广。Objective To explore the application effect of COOK balloon and Prostaglandin E2 Suppository in promoting cervical mature during induced labor of primiparas. Methods Eighty primiparas who were admitted to our hospital from January 2016 to June 2018 were selected as the study subjects. They were divided into experimental group and control group according to the lottery method, with 40 cases in each group. The experimental group used COOK balloon to promote cervical ripening, and the control group used Prostaglandin E2 Suppository to promote cervical ripening. The Bishop score before and after cervical ripening, total labor, postpartum hemorrhage, mode of delivery, and total incidence rate of complications were compared between the two groups. Results There was no significant difference in Bishop score between the two groups before cervical ripening (P>0.05). The Bishop scores of the two groups after cervical ripening were significantly higher than those before cervical ripening, and the differences were statistically significant (P<0.05). The Bishop score of the experimental group after cervical ripening was significantly higher than that of the control group, and the difference was statistically significant (P<0.05). The total labor in the experimental group was shorter than that in the control group, the postpartum hemorrhage was less than that in the control group, and the differences were statistically significant (P<0.05). The natural delivery rate of the experimental group was higher than that of the control group, and the difference was statistically significant (P<0.05). The total incidence rate of complications in the experimental group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion COOK balloon and Prostaglandin E2 Suppository have good effect on promoting cervical maturation. But the safety of COOK balloon promoting cervical mature is higher, it can decrease the incidence of complications such as neonatal asphyxia and improve delivery outc
