摘 要:目的:探讨近红外光谱(NIRS)检测脑血氧饱和度技术对新生儿脑缺氧的早期评估应用。方法:将我院2018年3月—2019年3月收治的43例明确诊断为新生儿缺氧缺血性脑病(Hypoxic-ischemic encephalopathy,HIE)的足产儿作为观察组,采用NIRS与脉搏血氧饱和度检测分别测定HIE患儿出生后3h、12h、48h的脑组织血氧饱和度(StO2)与脉搏血氧饱和度(SpO2),选择相同时间段出生的无合并HIE正常足月儿43例作为对照组,同样检测其出生后3h、12h、48h的StO2与SpO2,比较两组差异。结果:观察组HIE患儿出生后3h、12h、48h StO2均低于对照组,两组比较差异有统计学意义( P<0.01)。两组出生后3h、12h、48h SpO2比较差异没有统计学意义( P>0.05)。结论:NIRS检测脑血氧饱和度技术对HIE患儿的早期脑缺氧情况评估能够更加客观、准确,有助于对脑损伤的早期发现,提前介入临床指导,具有重要的临床意义。Objective:To explore the application of near infrared spectroscopy (NIRS) in early evaluation of neonatal cerebral hypoxia.Methods:43 full-term infants with hypoxic-ischemic encephalopathy (HIE) admitted to our hospital from March2018 to March2019 were selected as observation group. Cerebral tissue oxygen saturation (StO2) and blood oxygen saturation (SpO2) were measured 3 hours,12 hours and 48 hours after birth by NIRS and pulse oxygen saturation test,respectively.43 normal full-term newborns were taken as control group.The StO2 and SpO2 levels at 3 hours,12 hours and 48 hours after birth were also measured, and the differences between the two groups were compared.Results:The StO2 of the observation group at 3 hours,12 hours and 48 hours after birth,respectively were lower than that of the control group, and there was statistical difference between the two groups ( P <0.01).There was no significant difference in SpO2 between the two groups at 3 hours,12 hours and 48 hours after birth( P >0.05).Conclusion:NIRS can be used to detect cerebral oxygen saturation in early evaluation of HIE children more objectively and accurately.Brain hypoxia is conducive to early detection of brain injury and early intervention of clinical guidance, which has important clinical significance.
