中西医结合综合护理对糖尿病足患者临床观察

摘　　要：目的系统性评估中西医结合综合护理对糖尿病足患者的临床康复效果。方法选取自2017年6月—2018年6月于门诊及病房收治的糖尿病继发糖尿病足患者60例,入组病例均通过信封法随机分组。对照组30例,应用常规降糖对症治疗及西医护理措施;试验组30例,在对照组基础上联合中医汤药内服、外用及康复护理措施。观察2组患者的治疗前后神经缺陷NDS评分水平及感染率、周围神经电流感觉阈值CPT变化情况。结果临床护理后,试验组感染率:2例(6.67%),NDS评分(4.21±1.07)分,CPT阈值(342.51±58.06)mA×0.01。对照组:感染率:8例(26.67%),NDS评分(6.88±1.34)分,CPT阈值(261.19±69.29)mA×0.01,试验组均优于对照组,差异有统计学意义(P<0.05)。结论常规西医护理联合中医汤药内服及外用康复措施可有效降低并稳定糖尿病足患者血糖水平,降低局部感染及并发症发生率,改善患者周围循环及神经营养状态,提高患者自理能力及生活质量,值得临床推广。Objective To systematically evaluate the clinical rehabilitation effect of integrated Chinese and Western medicine on diabetic foot patients. Methods Sixty patients with diabetes secondary to diabetic foot admitted to our outpatient clinic and ward from June 2017 to June 2018 were enrolled. The enrolled patients were randomized by envelope method and 30 patients in the control group. Symptomatic treatment and Western medicine nursing measures, 30 cases in the experimental group, combined with traditional Chinese medicine decoction oral administration, external use and rehabilitation nursing measures on the basis of the control group, observed the neurological defect NDS score and infection rate before and after treatment, peripheral nerve current sensory threshold CPT Changes. Results After clinical care, the infection rate of the experimental group was 2 cases/6.67%, the NDS score was (4.21±1.07), and the CPT threshold was (342.51±58.06) mA×0.01. Control group: infection rate: 8 cases / 26.67%, NDS score (6.88±1.34), CPT threshold (261.19 ± 69.29) mA×0.01, the test group were better than the control group, and the difference had statistically significant (P<0.05). Conclusion The routine Western medicine nursing combined with traditional Chinese medicine decoction oral and external rehabilitation measures can effectively reduce and stabilize the blood sugar level of diabetic foot patients, reduce the incidence of local infection and complications, improve the peripheral circulation and neurotrophic state of patients, improve the patient's self-care ability and quality of life, It is worthy of clinical promotion.
