摘　　要：目的探讨钬激光联合等离子治疗男性后尿道狭窄对排尿功能的影响。方法选取2017年1月至2018年6月承德医学院附属医院收治的60例男性单纯性后尿道狭窄患者,应用随机数字表法分为联合组和对照组各30例,联合组应用钬激光联合等离子治疗,对照组应用单纯肾造瘘球囊扩张导管治疗,对比两组患者临床指标变化。结果两组住院时间、保留尿管时间差异无显著性(P>0.05);联合组手术时间、术中失血量、住院费用大于对照组,差异有显著性(P<0.05)。术前两组膀胱残余尿量、最大尿流率、生活质量评分差异无显著性(P>0.05);术后3个月,联合组最大尿流率高于对照组,膀胱残余尿量、生活质量评分低于对照组,差异均有显著性(P<0.05)。术后1个月,联合组并发症发生率20.00%,对照组为6.66%,差异无显著性(χ^2=2.308,P=0.129)。结论钬激光联合等离子治疗男性单纯性后尿道狭窄可以安全、有效的改善患者排尿功能。Objecive To investigate the effect of holmium laser combined with plasma on urination in male posterior urethral stricture.Method Total of 60 male patients with posterior urethral stricture admitted from January 2017 to June 2018 were randomly divided into the combined group and control group by using the number table method.The combined group was treated with holmium laser combined with plasma,while the control group was treated with pure balloon dilation catheter,the changes in clinical indicators of the two groups were compared.Result The difference in hospital stay and retention time between the two group was not significant(P>0.05);Operation duration,intraoperative blood loss and hospitalization cost of combined group were higher than control group(P<0.05).There was no significant difference in PVR,Qmax and QOL score between the two groups before operation(P>0.05);Three months after surgery,Qmax of the combined group was higher than the control group,and PVR,QOL score of the combined group were lower than the control group,with significant differences(P<0.05).One month after surgery,the incidence of complications in the combined group was 20.00%,compared with 6.66% in control group,with no significant difference(χ^2=2.308,P=0.129).Conclusion Holmium laser combined with plasma for the treatment of male posterior urethral stricture can safely and effectively improve the patients’ urination function.
