摘　　要：目的构建基于Strong高级护理实践模式的经外周中心静脉置管(peripherally inserted central catheter,PICC)专科护士管理方案,并评价其应用效果。方法针对PICC专科护士使用和管理中存在的问题,成立研究小组,以Strong高级护理实践模式为框架,构建PICC专科护士的使用和再认证管理方案,观察管理方案实施后PICC专科护士队伍的建设状况,质性访谈PICC专科护士自我体验,并比较方案实施前后全程跟踪病例PICC主要并发症的发生率。结果该管理方案可促进PICC专科护士队伍建设和专科护士职业成长,降低PICC并发症中置管穿刺点炎症、机械性静脉炎、导管外露大于6cm的发生率,与实施前相比,差异具有统计学意义(P<0.05)。结论基于Strong高级护理实践模式的PICC专科护士管理方案,有助于促进专科护理队伍建设,降低PICC相关并发症发生率,提高PICC临床护理质量。Objective To construct the management scheme for peripherally inserted central catheter (PICC) specialized nurses based on Strong advanced nursing practice model and to evaluate the application effect.Methods In view of the problems existing in the use and management of PICC nurse specialists, a research group was set up to construct a management scheme for the use and recertification of PICC nurse specialists based on the Strong advanced nursing practice model.The construction status of PICC nurse specialists' team after the implementation of the management scheme was observed,the self-experience of PICC nurse specialists was interviewed qualitatively and the incidence of major complications of PICC before and after the implementation of this program was compared.Results This management scheme can promote the team construction of PICC nurse specialists and the professional growth of nurse specialists.Compared with those before the implementation,the incidence of inflammation,mechanical phlebitis and catheter exposure longer than 6 cm was lower.The difference was statistical significance ( P <0.05).Conclusions The management scheme for PICC nurse specialists based on the Strong advanced nursing practice model is helpful to promote the team construction of nurse specialists,reduce the incidence of PICC-related complications and improve the nursing quality of clinical PICC care.
