应用数据挖掘分析周郁鸿教授治疗免疫性血小板减少症的症-药规律

摘　　要：目的挖掘周郁鸿教授辨证治疗免疫性血小板减少症的症-药规律。方法利用中医传承辅助系统(V2.5)软件,筛选出符合纳入标准的免疫性血小板减少症患者117例,共1102诊次临床资料,对包括主要症状、中医证候、中医治则治法、舌象、脉象及患者症-药关联几方面进行数据分析和挖掘。结果周郁鸿辨治免疫性血小板减少症的主要证候依次是气阴亏虚、气血失和、肝旺脾虚、阴虚火旺、血热动血、气血两虚、阴虚血热证等,主要治则依次为益气滋阴、和血宁络、调肝扶脾、滋阴降火、清热凉血、补益气血、滋阴凉血等。结论周郁鸿教授认为免疫性血小板减少症的病机主要为虚(气阴亏虚、气血两虚)、郁、热(虚火灼络,热毒伤络)及瘀为主,治疗上以益气滋阴、补益气血、调肝扶脾、和血宁络、滋阴凉血及清热凉血为主。Objective:To summarize Professor Zhou yuhong’s experience in treating immune thrombocytopenia disease(ITP).Methods:117 patient were screened out according to the inclusion criteria with the software of TCM inheritance assistant system(V2.5).The data was analyzed and excavated for the main symptoms,TCM syndromes,TCM treatment principles,treatment methods,tongue signs,pulse signs and relationship between patients’ symptoms and medicines.Results:The main syndromes of ITP in Zhou Yuhong's treatment included:deficiency of Qi and Yin,disharmony between Qi and blood,abundance in liver and deficiency in spleen,blood heat stirring blood,deficiency of Qi and blood,deficiency of Yin and blood heat.The main treatment followed by invigorating Qi and nourishing Yin,reconciling venation,harmonizing liver and assisting spleen,clearing away heat and cooling blood,tonifying Qi and blood,nourishing Yin and cooling blood.Conclusion:Professor Zhou Yuhong thinks that the main pathogenesis of ITP is deficiency of Qi and Yin,deficiency of Qi and blood,liver depression and spleen deficiency,deficiency fire burning collateral,toxic heat burning collateral,blood stasis and collateral damage.In her treatment,the main methods are invigorating Qi and nourishing Yin,reconciling venation,harmonizing liver and assisting spleen,clearing away heat and cooling blood,tonifying Qi and blood,nourishing Yin and cooling blood.
