摘  要:目的观察盆炎汤对急性盆腔炎模型大鼠细胞炎症因子、免疫功能及自由基代谢的影响。方法采用子宫内接种混合菌液法制备急性盆腔炎Wistar大鼠模型,将60只Wistar大鼠随机分为空白组、模型组及盆炎汤低、中、高剂量组和对照组,每组10只,空白组和模型组给予生理盐水灌胃,盆炎汤低、中、高剂量组分别给予1、2、3g/mL的盆炎汤煎液,对照组给予0.63g/mL的妇科千金片,每日灌胃1次,连续给药3周。HE染色观察组织病理学形态,ELISA法检查血清肿瘤坏死因子-α(TNF-α)、白细胞介素-2(IL-2)和白细胞介素-6(IL-6)水平,流式细胞术检测血清中CD4+、CD8+和NKT细胞含量,并比较丙二醛(MDA)、超氧化物歧化酶(SOD)及血液流变学指标。结果空白组宫腔形态正常,子宫内膜结构完整,细胞排列紧密整齐,宫壁各层分界清楚,无炎性细胞浸润;模型组宫腔形态失常,子宫壁增厚,组织结构紊乱,子宫内膜脱落缺失,各层分界不清,上皮细胞变性坏死,大量炎性细胞浸润;盆炎汤各剂量组和对照组组织结构基本清晰,无明显炎性渗出,少量炎性细胞浸润。模型组TNF-α和IL-6水平显著高于空白组,而IL-2低于空白组,盆炎汤各剂量组和对照组TNF-α和IL-6水平显著低于模型组,IL-2水平显著高于模型组,且盆炎汤中、高剂量组和对照组显著低于盆炎汤低剂量组(P<0.05)。模型组CD4+、CD8+和NKT细胞含量显著高于空白组,盆炎汤各剂量组和对照组CD4+、CD25+和Treg含量显著低于模型组(P<0.05)。模型组MDA水平显著高于空白组,而SOD水平显著低于模型组,盆炎汤各剂量组和对照组MDA水平低于模型组,而SOD水平高于模型组(P<0.05)。模型组血液流变学指标明显改变,盆炎汤各剂量组和对照组血液流变学指标较模型组显著改善(P<0.05)。结论盆炎汤对急性盆腔炎模型大鼠具有较好的防治作用,可降低细胞炎症因子水平,改善免疫功能、自由基代�Objective:To observe the effects of Penyan Decoction on cytokines,immune function and free radical metabolism in rats with acute pelvic inflammatory disease.Methods:The Wistar rat model of acute pelvic inflammatory disease was prepared by intrauterine inoculation mixed bacteria method.60 Wistar rats were randomly divided into the blank group,the model group,low-dose,medium-dose,high-dose groups of Penyan Decoction and the control group,10 rats in each.The blank group and the model group were given normal saline.The low-dose, medium-dose and high-dose groups were given 1 g/mL,2 g/mL and 3 g/mL,respectively.The control group was given 0.63 g/mL of Qianjin Tablets,once a day,3 weeks of continuous administration.Histopathological morphology was observed by HE staining.TNF-α,IL-2 and IL-6 levels were examined by ELISA.Flow cytometry was used to detect the levels of CD4+,CD8+ and NKT cells in serum,and MDA,SOD and hemorheological indexes were compared.Results:In the blank group,the uterine cavity was normal;the endometrium structure was intact;the cells were arranged tightly and neatly;the division of the uterine wall was clear and there was no inflammatory cell infiltration.In the model group,the uterine cavity was abnormal;the uterine wall was thickened;the structure was disordered;the endometrium was lost;the boundary of each layer was unclear;the epithelial cells degenerated and necrotized,and a large number of inflammatory cells infiltrated.The tissue structure of each dose group of Penyan Decoction and the control group was basically clear,with no obvious inflammatory exudation.Most of the structures were still clear,and a small amount of inflammatory cells infiltrated.The levels of TNF-alpha and IL-6 in the model group were significantly higher than those in the blank group,while the levels of IL-2 were lower.
