改良约束带对减少ICU病人非计划性拔管及约束不良事件发生的效果分析

摘　　要：目的:研究改良约束带的制作和使用方法,探讨其对减少重症病人非计划性拔管及约束不良事件的发生效果。方法:便利抽取住院符合条件的80例病人作为研究对象,随机分为观察组和对照组各40例,使用《ICU病人身体约束评估量表》对病人进行评估,对需要身体约束的重症病人进行约束。观察组使用改良的透气式约束带,对照组使用常规的海绵约束带,在约束结束后比较2组病人约束效果以及约束不良事件的发生情况,并比较2组护士对约束的满意度。结果:观察组在使用改良约束带期间发生1例非计划性拔管,对照组发生5例,差异无统计学意义(P>0.05);观察组约束不良事件的发生率小于对照组(P<0.01);观察组护士对约束的满意度高于对照组(P<0.01)。结论:改良的透气式约束带可以有效降低非计划性拔管发生率,减少约束不良事件的发生并减轻护士的工作量,提高护士对约束满意度。Objective:To study the making and using methods of modified restraint band,and explore its effects on reducing the occurrence of unplanned extubation and adverse events in severe patients. Methods:Eighty eligible hospitalized patients were conveniently selected,and randomly divided into the observation group and control group(40 cases each group).The patients were evaluated using the "ICU Patient Physical Constraint Assessment Scale",and the severe patients were binded with physical restraints.The improved breathable restraint band and conventional sponge restraint band were used in the observation group and control group,respectively.After the restraint ended,the restraint effects,occurrence rate of adverse events,and nurses′ satisfaction with constraint were compared between two groups. Results:A case with unplanned tubing in observation group and 5 cases with unplanned tubing in control group were found,and the difference of which between two groups was not statistically significant( P >0.05).The incidence rate of adverse events in observation group was less than that in control group( P <0.01),and the nurses′ satisfaction in observation group was higher than that in control group( P <0.01). Conclusions:The improved breathable restraint band can effectively reduce the incidence rate of unplanned tubing,occurrence rate of adverse events and workload of nurses,and improve the satisfaction of nurses.
