摘　　要：目的优化硬镜用于取石保胆手术治疗胆囊结石的流程,探索更加微创的保胆手术方式。方法选择我科收治的2 334例行硬镜取石保胆手术的患者为研究对象,统计患者手术类型、不同术式的临床相关指标及并发症发生情况。结果单孔组的手术时间、术中出血量、住院时间、胃肠功能恢复时间及术后积液引流量均优于两孔组及三孔组,且两孔组优于三孔组(P<0.05)。单孔组的局部感染发生率显著低于三孔组(P<0.05)。结论单孔法操作简单,对腹腔内干扰及创伤最小,患者术后恢复相对更快,更加符合微创理念。Objective To optimize the procedure of gallbladder-preserving cholecystolithotomy by rigid cholecystoscope,and to explore a more minimally invasive method of gallbladder-preserving cholecystolithotomy. Methods A total of 2 334 patients recived gallbladder-preserving cholecystolithotomy by rigid cholecystoscope in our department were selected as the study objects. The operation types, clinical relevant indicators and complication of different operative methods were counted. Results The operation time, intraoperative bleeding volume, hospital stay, recovery time of gastrointestinal function and drainage volume of effusion in the single-hole group were better than those in the two-hole group and the three-hole group, and those in the two-hole group were better than the three-hole group(P<0.05). The incidence of local infection in the single-hole group was significantly lower than that in the three-hole group(P <0.05). Conclusion The operation of single-hole method is simple, with minimal intra-abdominal interference and trauma, and the patients recover relatively faster after operation. This method is more in line with the concept of minimally invasive.
