不同时段泵注右美托咪定对胸科手术患者拔管时应激反应的影响

摘　　要：目的研究分析不同时段泵注右美托咪定对胸科手术患者拔管时应激反应的影响。方法将我院收治的100例胸科手术患者以随机数字表法分为4组,每组25例。a1组于术前、a2组于术中、a3组于术毕分别泵注右美托咪定,a4组泵注等容量的生理盐水。比较四组患者不同时间点的各项指标水平变化。结果 a1、a2、a3组的OAA/S评分、术后躁动评分均低于a4组,且a3组低于a1、a2组(P<0.05)。a3组的拔管时间长于a1、a2、a4组(P<0.05)。t3~t5时刻,a1、a2、a3组的AD、NE、CORT水平均低于a4组(P<0.05)。t1~t5时刻,四组患者的SpO2和RR无差异(P>0.05)。结论泵注右美托咪定可以减轻拔管时患者的负面情绪,在术前和术中泵注右美托咪定可以有效的减轻应激反应。Objective To study the effect of pumping dexmedetomidine at different time on stress response during extubation in patients undergoing thoracic surgery. Methods A total of 100 patients undergoing thoracic surgery admitted in our hospital were divided into four groups by random number table method, with 25 cases in each group. Dexmedetomidine was pumped in the a1 group before operation, in the a2 group during operation, in the a3 group after operation respectively,while the a4 group were injected with saline of equal volume. The levels changes of each index among the four groups at different time points were compared. Results The OAA/S scores and the postoperative restlessness scores of the a1 group,a2 group and a3 group were lower than those of the a4 group, and those of the a3 group were lower than the a1 and a2 group(P<0.05). The extubation time of the a3 group was longer than that of the a1, a2, a4 group(P<0.05). The levels of AD, NE and CORT in the a1, a2 and a3 group were lower than those in the a4 group at t3-t5(P <0.05). There were no significant differences in SpO2 and RR among the four groups at t1-t5(P>0.05). Conclusion Pumping dexmedetomidine can alleviate the negative emotions of patients during extubation, and pumping dexmedetomidine before and during surgery can effectively alleviate the stress response.
