摘　　要：目的比较开放式乳突根治术与软壁外耳道重建鼓室成形术对胆脂瘤型中耳炎患者听力康复的影响。方法回顾性分析我院2014年1月至2017年12月收治的79例(79耳)胆脂瘤型中耳炎患者的临床资料,根据手术方式的不同将患者分为试验组(38例)与对照组(41例)。试验组给予软壁外耳道重建鼓室成形术治疗,对照组给予开放式乳突根治术治疗。比较两组患者的治疗效果。结果试验组患者的治疗总有效率显著高于对照组(P<0.05);治疗后,两组患者的气骨导差与气导听阈均降低,且试验组显著低于对照组(P<0.05);试验组患者的术后干耳所需时间显著短于对照组,术后1年复发率显著低于对照组(P<0.05)。结论软壁外耳道重建鼓室成形术治疗胆脂瘤型中耳炎的效果显著,可显著改善患者的听力,术后干耳快,复发率低。Objective To investigate the effect of open radical mastoidectomy and tympanoplasty with soft-wall reconstruction of ear canal on hearing rehabilitation in patients with cholesteatoma otitis media. Methods The clinical data of 79 cases(79 ears) of cholesteatoma otitis media treated in our hospital from January 2014 to December 2017 were retrospectively analyzed. The patients were divided into experimental group and control group according to different surgery methods. The experimental group was given tympanoplyasty with soft-wall reconstruction of ear canal, the control group was given open radical mastoidectomy. The therapeutic effects of the two groups were compared. Results The total effective rate of treatment in the experimental group was significantly higher than that in the control group(P<0.05). After treatment, the air-bone gap and air-guided hearing threshold of the two groups decreased significantly, and those in the experimental group were significantly lower than the control group(P<0.05). The time of dry ear after surgery in the experimental group was significantly shorter than that in the control group, and the recurrence rate at 1 year after surgery was significantly lower than that in the control group(P<0.05). Conclusion The tympanoplasty with soft-wall reconstruction of ear canal in patients with cholesteatoma otitis media has a significant effect, which can significantly improve the hearing of patients with fast dry ear and low recurrence rate.
