简易膀胱容量压力测定法联合排尿日记对缺血性脑卒中患者拔除导尿管时机选择的影响

摘　　要：目的:探讨简易膀胱容量压力测定法联合排尿日记对缺血性脑卒中患者拔除导尿管时机选择的影响.方法:采用随机数字表法将2016年1月1日~2017年6月30日80例缺血性脑卒中留置导尿管患者分为对照组和观察组各40例,对照组给予基础护理,观察组给予简易膀胱容量压力测定法联合排尿日记;比较两组拔除导尿管后首次顺利排尿时间、首次排尿容量、再次插管率,排尿2次后膀胱残留尿量及排尿异常发生率.结果:观察组首次顺利排尿时间明显短于对照组(P<0.05),首次排尿容量大于对照组(P<0.05),再次插管率低于对照组(P<0.05),患者尿2次后膀胱残留尿量少于对照组(P<0.05),尿潴留、尿失禁、尿频总发生率低于对照组(P<0.05).结论:对缺血性脑卒中患者采用简易膀胱容量压力测定法联合排尿日记可为医护人员评估患者膀胱功能提供参考,为确定拔除导尿管时机提供依据,有助于降低再次插管率.Objective:to investigate the effect of simple bladder volume pressure measurement combined with urination diary on timing of catheter extraction in patients with ischemic stroke.Methods:A total of 80 patients with ischemic stroke with indwelling catheter from January 1,2016 to June 30,2017 were divided into control group and observation group with 40 patients each.The control group was given basic nursing,and the observation group was given simple bladder volume pressure measurement combined with urination diary.The time of the first successful urination,the volume of the first urination,the rate of re-intubation,the residual urine volume of the bladder and the incidence of abnormal urination after urinating twice were compared between the two groups.Results:The smooth urination time of the first time in observation group was significantly shorter than the control group( P <0.05),the first urination capacity was greater than the control group( P <0.05),the re-intubation rate was less than the control group( P <0.05),and bladder residual urine volume after twice was less than the control group( P <0.05).The urinary retention,urinary incontinence,and total rate of frequency of urination in observation group was lower than those in the control group( P <0.05).Conclusion:simple bladder volume pressure measurement combined with urination diary in patients with ischemic stroke can provide a reference for medical staff to evaluate bladder function in patients,provide a basis for determining the timing of catheter removal,and help to reduce the rate of reintubation.
