基于“血三脏”与脑肠轴论治肠易激综合征

摘　　要："血三脏"(与血的生成、贮藏和运行关系密切相关的心、肝、脾三脏)的生理功能失常会影响大肠传导功能,从而出现便秘或腹泻,导致肠易激综合征(IBS)的产生;胃肠道的消化、吸收及排泄废物等功能有赖于脑肠轴(中枢神经、肠神经和自主神经所组成的调控系统)的调节,脑肠轴功能失调亦可引发IBS。"血三脏"理论可为IBS的治疗提供思路,而脑肠轴学说可为中医治疗IBS的疗效机制研究提供理论基础。"血三脏"理论与脑肠轴学说虽分属中、西医学两个不同的医学理论体系,但两者均强调情志因素在IBS发病过程中的作用,故治疗时应注重调节患者情志,主张心肝脾三脏同治,采用疏肝健脾、宁心安神法治疗IBS。The dysfunction of blood triple Zang-organs of heart,liver,and spleen,which are closely related with the formation,storage and transportation of blood,can affect the transmission function of large intestine and induce constipation and diarrhea,and can result into the occurrence of irritable bowel syndrome(IBS). Brain-gut axis, a regulatory system consists of central nervous system, enteric nervous system and autonomic nervous system,plays an important role in the regulation of gastrointestinal digestion and absorption and waste discharge.The dysfunction of brain-gut axis can also result into IBS. The theory of blood triple Zang-organs can supply thoughts for Chinese medicine therapy for IBS, and brain-gut axis can supply evidence for the therapeutic mechanism of Chinese medicine therapy for IBS. Although blood triple Zang-organs and brain-gut axis belong to different fields of medical theory, both of them emphasize the important role of emotional factors in the pathogenesis process of IBS. Therefore, the treatment for IBS should stress the emotional regulation and simultaneious regulation of heart,liver and spleen, and the therapies of soothing liver and invigorating spleen,and tranquilizing mind can be adopted.
