摘 要:目的:调查南京市育龄期妇女妊娠期糖尿病(gestational diabetes mellitus,GDM)发病率,以期获得南京地区GDM的流行现况。方法:收集2016年4月~2016年8月进行产检的5712例孕24~28w周孕妇的口服葡萄糖耐量试验(oral glucose tolerance test,OGTT)结果,GDM诊断参照2014年国际糖尿病与妊娠研究组制定的GDM诊断标准判定。随后按孕妇年龄分为20~24岁(A组),25~29岁(B组),30~34岁(C组)和≥35岁(D组) 4组,分别计算各组空腹血糖(fasting plasma glucose,FPG)、OGTT 1小时糖(OGTT-1h)、OGTT 2小时糖(OGTT-2h)水平及GDM发病率。结果:随着年龄增长,孕妇FPG、OGTT-1h及OGTT-2h水平不断增高,差异具有明显的统计学意义(FPG:H=37.30,P<0.0001;OGTT-1h:H=202.9,P<0.0001;OGTT-2h:H=185.9,P<0.0001);本次调查显示GDM患病总人数为1203人,总发病率为21.06%,A、B、C、D组GDM检出率为别为14.72%、17.72%、25.21%和32.12%,不同年龄组GDM发病率具有明显的统计学差异(x^2=89.10,P<0.0001)。结论:南京地区GDM发病率处于较高水平,尤其是30岁以上孕妇的GDM发病明显增加。Objective:The aim of this study is to investigate the prevalence of gestational diabetes mellitus(GDM) among women at reproductive age in Nanjing. Methods:The results of 5712 oral glucose tolerance test(OGTT) of pregnant women at a gestational age of 24~28 weeks who were examined in the Department of Obstetrics of the Obstetrics and Gynecology Hospital Affiliated to Nanjing Medical University from April 2016 to August 2016 were collected. The GDM was diagnosed according to International association of diabetes and pregnancy study groups(IADPSG). Subsequently, they were divided into four groups according to the age of pregnant women: 20~24 years old(group A), 25~29 years old(group B), 30~34 years old(group) and ≥ 35 years old(group D). Lastly, the level of fasting Plasma glucose(FPG), OGTT 1 hour sugar(OGTT-1 h), OGTT 2 hour sugar(OGTT-2 h) and GDM incidence rate of each group were analyzed. Results:The levels of FPG, OGTT-1 h and OGTT-2 h in pregnant women increased continuously with the increase of age and there were statistically significant differences between the comparing groups(FPG:H=37.30,P<0.0001;OGTT-1 h:H=202.9,P<0.0001;OGTT-2 h:H=185.9,P<0.0001). This study showed that the total number of GDM patients was 1203 and the total incidence rate of GDM was 21.06%. The detection incidence rates of GDM in groups A, B, C and D were 14.72%, 17.72%, 25.21% and 32.12% respectively. There was a statistically significant difference in the incidence rate of GDM between the groups(x^2=89.10, P<0.0001).Conclusion:The incidence rate of GDM in Nanjing was at a high level, especially in the pregnant women who were older than 30 years old.
