马骏运用宏观辨证结合胃黏膜相治疗慢性萎缩性胃炎经验

摘　　要：马骏教授据多年临证经验,将慢性萎缩性胃炎宏观辨证为肝胃不和证、饮食积滞证、痰瘀互结证、寒热错杂证、脾胃虚寒证、胃阴不足证。马教授以宏观辨证为基础,结合胃黏膜相,治疗慢性萎缩性胃炎疗效显著。胃黏膜病理形态学改变在慢性萎缩性胃炎辨病与辨证中具有决定性作用。胃黏膜相在判断病因、病机、病性、病势中具有指引作用。胃黏膜相可以弥补宏观辨证论治之不足,进一步明确诊断,提高临床疗效。以"调和致中"为指导思想,以"权衡、寒热、升降、润燥、通补"为具体治疗原则,宏观辨证与胃黏膜相结合,应用先进的技术手段进一步加强对胃黏膜相的研究,才能不断丰富诊疗手段,提高诊断与治疗水平,改善患者生活质量,预防胃癌的发生与发展。Professor Ma Jun has classified chronic atrophic gastritis as liver-stomach disharmony syndrome, diet stagnation syndrome, phlegm-blood stasis syndrome, syndrome of cold and heat in complexity, spleen-stomach deficiency-cold syndrome and stomach-yin deficiency syndrome according to his own many years of clinical experience. Professor Ma, based on macro-syndrome differentiation and combined with gastroscopic gastric mucosa, has achieved a remarkable therapeutic effect on chronic atrophic gastritis. Pathomorphological changes of gastric mucosa play a decisive role in the disease differentiation and syndrome differentiation of chronic atrophic gastritis. Gastric mucosal phase plays a guiding role in judging etiology, pathogenesis, disease nature and disease situation. Gastric mucosal phase can make up for the deficiency of macroscopic syndrome differentiation and treatment, further clarify the diagnosis and improve the clinical efficacy. We should take "harmonizing" as the guiding ideology, take "balance, cold and heat, ascending and descending, moistening and drying, dredging and tonifying" as the specific treatment principle, combine macroscopic syndrome differentiation with gastric mucosa phase, and further strengthen the study of gastric mucosal phase by using advanced technical means, so as to continuously enrich the means of diagnosis and treatment, improve the level of diagnosis and treatment, and enhance the quality of life of patients and prevent the occurrence and development of gastric cancer.
