摘 要:目的评价线性探针法在四川省乐山市结核耐药性检测中的应用效果。方法收集2017-04/2019-02乐山各县区疾控中心结核病实验室痰培养获得的270株结核菌株作为研究对象,同时采用线性探针法和标准方法比例法检测结核分枝杆菌对异烟肼和利福平的耐药情况,采用一致性检验和χ2检验进行分析,计算Kappa值评价线性探针法和比例法结果的一致性,以P<0.05为差异具有统计学意义。结果 270株结核菌株标本中,使用线性探针法检测异烟肼耐药性的灵敏度为84.00%,特异度为97.54%,阳性预测值为77.78%,阴性预测值为98.35%,Kappa值为0.79;对利福平耐药性的灵敏度为95.83%,特异度为96.34%,阳性预测值为71.88%,阴性预测值为99.58%,Kappa值为0.80;对耐多药结核的灵敏度为80.00%,特异度为98.03%,阳性预测值为80.00%,阴性预测值为98.81%,Kappa值为0.73。与标准方法比例法相比,两者具有高度的一致性。结论线性探针法具有高度的特异性和敏感性,且高效、快速,能有效的弥补标准方法的不足,为乐山市耐药结核病的早发现、早治疗提供了有力的保障。Objective To evaluate the application of line probe assay in the detection of drug resistant of tuberculosis in Leshan city, Sichuan province. Methods There were 270 tuberculosis strains which were collected from sputum culture in tuberculosis laboratories of the Center for Disease Control and Prevention of all counties and districts in Leshan during April 2017 to Feb. 2019. Line probe assay and standard proportion method were used to detect the resistance of mycobacterium tuberculosis to isoniazide and rifampicin, consistency test and χ2 test were used to analysis. The concordance of Line probe assay and proportion method was evaluated with Kappa value, the differences of P<0.05 were considered to be statistically significant. Results Among the 270 tuberculosis strains, the detection of isoniazide-resistance by line probe assay had a sensitivity,specificity, positive predictive values, negative predictive value and Kappa value, they were84.00%, 97.54%, 77.78%, 98.35% and 0.79,respectively;Detection of rifampicin-resistance by line probe assay had sensitivity, specificity,positive and negative predictive values, and Kappa value, they were 95.83%, 96.34%,71.88%, 99.58% and 0.80, respectively;Detection of multidrug-resistant tuberculosis by line probe assay had a sensitivity, specificity, positive and negative predictive values and Kappa value, they were 80.00%, 98.03%, 80.00%, 98.81% and 0.73, respectively. There was a high degree of consistency compared with the standard method. Conclusion Line probe assay had high specificity and sensitivity with efficiency,it was making up the deficiency of the standard method.It strongly guaranteed the early detection and treatment of drug-resistant tuberculosis in Leshan city.
