摘 要:目的:分析我院儿童急性呼吸道感染的非细菌性病原体分布特点,为呼吸道病原体预防控制提供依据.方法:采用间接免疫荧光法呼吸道病原体检测试剂盒,检测1238例诊断为急性呼吸道感染的患儿血清学标本中的8种非细菌性病原体IgM抗体,包括腺病毒(ADV)、肺炎衣原体(CP)、流感病毒A型(INFA)、流感病毒B型(INFB)、肺炎支原体(MP)、副流感病毒(PIV)、呼吸道合胞病毒(RSV)和嗜肺军团菌(LP),依据不同年龄和季节分组,分析各组呼吸道病原体检出率差异.结果: 1238例患者中呼吸道病原体IgM抗体总检出率为39.74%,其中检出率最高是肺炎支原体,其次是流感病毒B型和呼吸道合胞病毒;儿童急性呼吸道病原体IgM抗体不同性别间检出率差异无统计学意义,呼吸道病原体检出率在不同年龄组间差异有统计学意义,其中学龄组最高,其次为学龄前组,此两组检出率均高于婴儿组和幼儿组(P<0.05),呼吸道病原体检出率不同季节间差异有统计学意义,春季和冬季显著高于夏季和秋季(P<0.05).结论:我院儿童呼吸道感染病原体以肺炎支原体、流感病毒B型和呼吸道合胞病毒为主.各病原体检出率存在年龄和季节分布差异.Objective: To analyze the epidemiological characteristics of respiratory tract infections in children with acute respiratory tract infections in our hospital so as to provide basis for prevention and control of these pathogens. Methods: The serum IgM antibody of eight species of respiratory tract pathogens including Adenovirus (ADV), Chlamydia pneumoniae (CP), Influenza A (INFA), Influenza B (InFB), Mycoplasma pneumoniae (MP), Parainfluenza virus (PIV), Respiratory syncytial virus (RSV) and Legionella from 1238 children patents were examined by Indirect immunofluorescence assay. Results were statistically analyzed according to the gender, age and season. Results: Out of the 1238 cases,the total positive rate of respiratory tract pathogens was 39.74%. MP have the highest positive rate, followed by INFB and RSV. There is no difference between different gender groups (P>0.05). 6-14 age group had the highest positive rate and the 3-6 age group was the second highest. Both of them have a higher positive rate than the other two age groups (P<0.05). The positive rates of IgM were significantlydifferentbetween seasons. The positive rate was higher in spring and winter than in summer and autumn (P<0.05). Conclusion: The main respiratory tract infection pathogens in children with acute respiratory tract infections are MP, INFB and RSV in Jinan. The positive rate between different age and season groups is different.
