人工股骨头置换治疗老年股骨转子间不稳定型骨折的短期效果观察

摘　　要：目的探讨Ⅰ期人工股骨头置换治疗老年股骨转子间不稳定型骨折的效果、预后及其适应证.方法2013年1月至2017年1月采用生物型人工股骨头假体对25例老年股骨转子间不稳定型骨折患者行置换手术.术后定期随访,复查髋部X线,并以髋关节Harris评分评估髋关节功能.结果所有置换手术均由同一位经验丰富的医师完成,随访时间均>12个月.术后1、3、6、12个月髋关节评分分别为:77.8±7.8(65~91),78.0±7.6(60~92),78.4±7.0(64~90),78.8±7.6(60~90),随访期间所有患者均未发生假体松动、脱位、假体周围骨折等并发症.结论人工股骨头置换治疗老年股骨转子间不稳定型骨折的效果肯定,但需严格掌握手术适应证,且病例数及随访时间仍较少,尚需进一步随访研究.Objective To study the effect,prognosis and indication of the artificial femoral head replacement for the treatment of unstable intertrochanteric fracture in aged patients. Methods From January 2013 to January 2017,25 patients with unstable intertrochanteric fractures were treated with artificial femoral head replacement in Taizhou Central Hospital.After operation,the hip X-ray was reviewed regularly and hip joint function was evaluated by hip Harris score. Results All the patients were operated successfully by the same experienced doctor,the follow-up time was more than 12 months.The hip Harris score were 77.8±7.8(65~91),78.0±7.6(60~92),78.4±7.0(64~90),78.8±7.6(60~90)in 1 month,3months 6 months 12 months after operation respectively.None of the patients had loose or dislocation of prothesis,periprosthetic fractures. Conclusion Artificial femoral head replacement for unstable intertrochanteric fractures in aged patients is effective in the recovery of hip joint function .But the indications need to be strictly controlled.
