观察厄贝沙坦联合氨氯地平对老年高血压患者血压变异性的影响

摘　　要：目的 观察厄贝沙坦联合氨氯地平对老年高血压患者血压变异性的影响。方法 66例老年高血压患者,随机分为观察组和对照组,各33例。观察组给予厄贝沙坦联合氨氯地平治疗,对照组给予厄贝沙坦治疗。观察比较两组患者的临床疗效及治疗前后血压变异性、血管内皮细胞功能。结果 观察组治疗总有效率为93.94%,明显高于对照组的72.73%,差异有统计学意义(P<0.05)。治疗后,观察组24h平均舒张压变异性(7.4±2.2)%、24h平均收缩压变异性(9.4±2.4)%低于对照组的(10.5±2.1)%、(13.7±3.5)%比较差异均具有统计学意义(P<0.05)。治疗后,观察组一氧化氮水平(54.2±9.8)μmol/L高于对照组的(44.5±7.3)μmol/L,内皮素水平(32.1±5.2)ng/L低于对照组的(36.2±8.5)ng/L,差异均具有统计学意义(P<0.05)。结论 给予老年高血压患者厄贝沙坦联合氨氯地平治疗,能提高治疗效果,降低血压变异性,改善血管内皮细胞功能,值得借鉴。Objective To observe the effect of irbesartan combined with amlodipine on blood pressure variability in elderly patients with hypertension. Methods A total of 66 elderly patients with hypertension were randomly divided into observation group and control group, with 33 cases in each group. The observation group was treated with irbesartan combined with amlodipine, and the control group was treated with irbesartan. Observation and comparison were made on clinical efficacy, blood pressure variability and vascular endothelial cell function before and after treatment between the two groups. Results The observation group had obviously higher total effective rate of treatment as 93.94% than 72.73% in the control group, and the difference was statistically significant (P<0.05). After treatment, 24 h mean diastolic pressure variability (7.4±2.2)%, 24 h mean systolic blood pressure variability (9.4±2.4)% in the observation group was lower than (10.5±2.1)% and (13.7±3.5)% in the control group, and their difference was statistically significant (P<0.05). After treatment, The level of nitric oxide (54.2±9.8)μmol/L in the observation group was higher than (44.5±7.3)μmol/L in the control group, and level of endothelin (32.1±5.2) ng/L was lower than (36.2±8.5) ng/L in the control group. Their difference was statistically significant (P<0.05). Conclusion Combination of irbesartan and amlodipine can improve the therapeutic effect of elderly patients with hypertension, reduce blood pressure variability and improve vascular endothelial cell function. It is worthy of reference.
