血清AFU、IL-17、CRP在乙型病毒性肝炎患者中的检测价值

摘　　要：目的分析血清a-L-岩藻糖苷酶(AFU)、白细胞介素17(IL-17)、C反应蛋白(CRP)水平在诊断乙型肝炎病毒(HBV)感染及判断病情中的意义。方法选取2015年1月至2018年6月该院收治的272例HBV感染患者纳入研究组,根据病程进展进一步分为慢性乙型肝炎组128例、代偿期肝硬化组78例、失代偿期肝硬化组66例。并选择同期该院66例体检健康者纳入对照组。采用ELISA检测各组研究对象血清中AFU、IL-17水平,采用免疫比浊法检测CRP水平,并进行比较。结果AFU、IL-17、CRP水平在对照组、慢性乙型肝炎组、代偿期肝硬化组、失代偿期肝硬化组中依次升高,两两比较,差异均有统计学意义(P<0.05)。在HBV感染患者中,AFU水平与CRP水平呈低度相关(r=0.315,P<0.001),IL-17水平与CRP水平呈中度相关(r=0.563,P<0.001)。受试者工作特征曲线显示,AFU、IL-17诊断HBV感染的灵敏度和特异度分别为100.00%、67.65%,100.00%、70.22%。结论乙型病毒性肝炎患者血清AFU、IL-17、CRP水平随病情加重而升高。Objective To analyze the significance of serum levels of a-L-fucosidase(AFU),interleukin-17(IL-17)and C-reactive protein(CRP)in the diagnosis of hepatitis B virus(HBV)infection and judgement disease condition.Methods A total of 272 cases infected HBV in Orthopedics Hospital of Ruzhou City from January 2015 to June 2018 were selected in study group.According to disease condition,272 subjects were divided as 128 cases in chronic hepatitis B group,78 cases in compensatory cirrhosis group and 66 cases in decompensated cirrhosis group.Other 66 healthy person were recruited into control group.The serum levels of AFU,IL-17 were measured by ELISA,and CRP was detected by immunoturbidimetry assay,then compared these indicators in these groups.Results AFU,IL-17 and CRP levels increased successively in the control group,chronic hepatitis B group,compensatory cirrhosis group and decompensated cirrhosis group,and the differences of two groups were statistically significant(P<0.05).In patients infected with HBV,AFU level correlated mildly with CRP level(r=0.315,P<0.001),and IL-17 level correlated moderately with CRP level(r=0.563,P<0.001).Subject operating characteristic curves showed that the sensitivity and specificity of AFU and IL-17 in diagnosing HBV infection were 100.00%,67.65%and 100.00%,70.22%respectively.Conclusion Serum AFU,IL-17 and CRP increased with the aggravation of disease condition of HBV infection.
