参芪益心方治疗慢性心力衰竭的临床疗效观察

摘　　要：目的观察参芪益心方治疗慢性心力衰竭的临床疗效。方法选取2018年10月—2019年4月就诊于安徽中医药大学第一附属医院心内科的慢性心力衰竭病人40例,随机分为对照组和治疗组,每组20例。对照组采用常规西药治疗;治疗组采用参芪益心方加常规西药治疗。观察两组治疗前后中医证候积分、心功能分级、B型脑钠肽(BNP)、左心室舒张末内径(LVEDd)、左心室射血分数(LVEF)、明尼苏达州心力衰竭生活质量(MLHFQ)评分等指标变化情况。结果治疗4周后,治疗组中医证候疗效总有效率为90.0%,高于对照组的70.0%,差异有统计学意义(P<0.05),治疗组心功能疗效总有效率为90.0%,高于对照组的65.0%,差异有统计学意义(P<0.05);两组治疗后MLHFQ评分、BNP水平较治疗前下降(P<0.05),且治疗组低于对照组(P<0.05);与治疗前比较,两组LVEDd均降低,但差异无统计学意义(P>0.05);与治疗前比较,两组LVEF升高,差异有统计学意义(P<0.05);治疗后,治疗组LVEF高于对照组(P<0.05)。两组均未出现严重不良反应。结论参芪益心方可以减轻慢性心力衰竭病人临床症状,改善病人心功能及生活质量。Objective To observe the clinical effect of Shenqi Yixin prescription in the treatment of chronic heart failure.Methods Forty patients with chronic heart failure were selected and randomly divided into treatment group(Shenqi Yixin prescription plus conventional western medicine treatment)and control group(conventional western medicine treatment)with 20 cases in each group.The TCM syndrome efficacy,New York Heart Association(NYHA)heart function classification,B-type brain natriuretic peptide(BNP),left ventricular end diastolic diameter(LVEDd),left ventricular ejection fraction(LVEF),Minnesota heart failure quality of life(MLHFQ)scores were observed.Results After four weeks treatment,the total effective rates of TCM syndrome(90.0%)in the treatment group was higher than that in the control group(70.0%,P<0.05);the total effective rates of cardiac function(90.0%)in the treatment group was higher than that in the control group(65.0%,P<0.05).The MLHFQ score and BNP level in the two groups were lower than those before treatment(P<0.05),and the MLHFQ score and BNP level in the treatment group were lower than those in the control group(P<0.05).The LVEDd in the two group was lower than that before treatment,the difference was not statistically significant(P>0.05).The LVEF was lower than that before treatment(P<0.05),and the LVEF in the treatment group was higher than that in the control group.No adverse reactions occurred in both groups.Conclusion Shenqi Yixin prescription can relieve clinical symptoms of patients with chronic heart failure,and improve cardiac function and quality of life.
