摘 要:目的研究妊娠期糖尿病(gestational diabetes mellitus)的患病率和危险因素。方法采用横断面的研究方法,收集2015年1月1日至2017年12月31日期间河北省22所监测医院入院分娩的156 973例孕产妇资料,分析妊娠期糖尿病的患病率及其危险因素。结果 156 973例孕产妇中诊断妊娠期糖尿病者8 498例(5.4%),低于我国平均水平;2015-2017年其患病率呈现逐年增长趋势,平均患病年龄(28.54±4.59)岁,患病率随着年龄的增加而增长,35岁以上孕产妇患病率达9.7%;保定孕产妇妊娠期糖尿病患病率为13.12%,张家口为0.09%。多因素回归分析显示以下因素是妊娠期糖尿病的独立危险因素:妊娠合并慢性高血压(OR=2.07,95%CI:1.76~2.44),轻度子痫前期(OR=2.10,95%CI:1.75~2.51),重度子痫前期(OR=1.56,95%CI:1.37~1.77),孕次(OR=1.19,95%CI:1.13~1.26)和剖宫产史(OR=1.36,95%CI:1.29~1.43)。结论妊娠期糖尿病患病率随着年龄的增加而增长,建议适龄孕育,针对不同人群,尤其是高危人群进行早期筛查,采取预防措施,加强监测,对改善围产结局具有重要意义。Objective To understand the prevalence and risk factors of gestational diabetes mellitus.Methods The prevalence and risk factor of gestational diabetes were analyzed in a cross-sectional study that collected 156 973 maternal data from 22 hospitals in Hebei Province during the period from January 1,2015 to December 31,2017. Results Of the 156 973 pregnant women,8 498(4.0%)were diagnosed with gestational diabetes,which was lower than the average value in China. The prevalence rate increased from 2015 to 2017,with an average age of(28.54 ± 4.59)years old. The prevalence increased with age,and of over 35 years old was over 9.7%. The prevalence of gestational diabetes in Baoding and Zhangjiakou was 13.12% and 0.09% respectively.Multivariate regression analysis showed that the following factors were independent risk factors for gestational diabetes:pregnancy with chronic hypertension(OR = 2.07,95% CI:1.76 ~ 2.44),mild pre-eclampsia(OR =2.10,95% CI:1.75 ~ 2.51),severe pre-eclampsia(OR = 1.56,95% CI:1.37 ~ 1.77),pregnancy(OR = 1.19,95% CI:1.13 ~ 1.26)and history of cesarean section(OR = 1.36,95% CI:1.29 ~ 1.43). Conclusion The prevalence of gestational diabetes increases with age. It is recommended to breed at an appropriate age. Early screening for different populations,especially high-risk groups,taking preventive measures and strengthening monitoring are of great significance for improving perinatal outcomes.
