摘 要:目的探究2型糖尿病(type 2 diabetes,T2DM)和糖尿病肾病(diabetic nephropathy,DN)患者血清miR-181b水平及意义。方法选取2016年5月至2017年4月的87例T2DM并发DN患者作为研究对象。采用RT-qPCR法检测血清miR-181b水平,并分析其与T2DM进展为DN及DN预后的关系。结果 DN组血清miR-181b水平高于T2DM组和对照组(P <0.05)。多元线性回归分析结果显示收缩压和ACR与miR-181b独立相关。血清miR-181b诊断T2DM进展为DN的ROC AUC、敏感度和特异度分别为0.911、71.26%和97.62%。miR-181b高水平组有28例进展为终末期肾病(end stage renal disease,ESRD),miR-181b低水平组有8例进展为ESRD,差异有统计学意义(P <0.05)。结论 T2DM和DN患者血清miR-181b水平升高。miR-181b与T2DM、DN患者收缩压和ACR呈独立相关。检测miR-181b水平有助于DN诊断及预后评估。Objective To investigate the serum levels of miR-181b in patients with type 2 diabetes mellitus(T2DM)and diabetic nephropathy(DN). Methods Eighty-seven patients with T2DM and complication of DN presented to our hospital from May 2016 to April 2017 were selected as subjects. The serum level of microRNA-181b was measured by RT-qPCR. Its relationship with the progression of T2DM to DN and the prognosis of DN was analyzed. Results The serum miR-181b level in the DN group was higher than that in the T2DM group and the control group(P < 0.05). Multiple linear regression analysis showed that both systolic blood pressure and ACR were independently associated with miR-181b. The ROC AUC,sensitivity and specificity of serum miR-181b for the diagnosis of T2DM to DN were 0.911,71.26% and 97.62%,respectively. A total of 28 patients in the high-level miR-181b group progressed to ESRD,and 8 patients in the low-level miR-181b group progressed to ESRD. The difference was statistically significant(P < 0.05). Conclusions Serum miR-181b levels are elevated in patients with T2DM and DN. miR-181b is independently associated with systolic blood pressure and ACR. Detection of serum miR-181b levels contributes to the diagnosis and prognosis of DN.
