摘　　要：目的:探讨阿法替尼治疗非小细胞肺癌患者致严重的药品不良反应(adverse drug reaction,ADR)的相关危险因素,为临床安全、有效使用阿法替尼,预防ADR提供参考。方法:回顾性分析2017年至2018年6月四川省肿瘤医院96例使用过阿法替尼且发生ADR的非小细胞肺癌患者的临床病历资料,对可能引起阿法替尼致严重的ADR的危险因素进行多因素、非条件Logistic回归分析。结果:96例服用阿法替尼患者的单因素分析结果显示,是否有内科合并症和近期是否化疗等2个因素对阿法替尼致严重的ADR发生均有显著影响(P<0.05);对筛选出的影响因素进行非条件Logistic回归分析,结果显示,近期是否化疗和内科并发症等2个因素对阿法替尼致严重的ADR发生均有显著影响(P<0.05)。结论:非小细胞肺癌患者使用阿法替尼会出现严重的ADR,尤其是近期化疗和有内科合并症者;一旦发生严重的ADR,应积极对症治疗。OBJECTIVE:To investigate the related risk factors of severe adverse drug reactions (ADR) in patients with non-small cell lung cancer treated with afatinib,so as to provide reference for safe and effective use of alfaitinib and prevention of ADR.METHODS:Retrospective analysis was conducted on the clinical records of 96 patients with non-small cell lung cancer who had used alatinib and developed ADR in Sichuan Cancer Hospital from 2017 to Jun.2018,and the risk factors of severe ADR caused by alatinib were analyzed by multivariate and unconditional Logistic regression.RESULTS:Univariate analysis results of 96 patients taking alfaitinib showed that the presence of medical complications and recent chemotherapy had significant effects on severe ADR caused by alfaitinib ( P <0.05).Unconditional Logistic regression analysis was conducted on the screened influencing factors,and the results showed that the two factors,including recent chemotherapy and medical complications,had significant influences on severe ADR caused by alfaitinib ( P <0.05).CONCLUSIONS:Clinical application of afatinib can lead to severe ADR,especially in patients with recent chemotherapy and medical complications.Patients with severe ADR should be actively treated with symptomatic treatment.
