摘　　要：目的:探讨静脉药物配置中心(pharmacy intravenous admixture services,PIVAS)药师对抗肿瘤药不合理医嘱的干预效果,确保临床合理用药。方法:对2014—2018年北京大学肿瘤医院(以下简称“我院”)PIVAS药师干预的抗肿瘤药不合理医嘱进行统计和回顾性分析。结果:2014—2018年,我院PIVAS共接收381080条抗肿瘤药医嘱,其中不合理医嘱2014条,医嘱总不合理率为0.53%。PIVAS药师干预成功率由2014年的99.36%(467/470)升至2018年的100.00%(379/379),抗肿瘤药医嘱不合理率由2014年的0.74%(470/63115)降至2018年的0.40%(379/93766)。抗肿瘤药不合理医嘱主要包括溶剂用量不合理、医嘱提交不合理及溶剂选择不合理等6种类型。结论:我院PIVAS药师对抗肿瘤药不合理医嘱的干预效果显著,有效降低了药物的不合理应用,临床用药的有效性、安全性和经济性在很大程度上得到了保证。OBJECTIVE:To probe into the intervention of unreasonable use of anti-tumor drugs in Pharmacy Intravenous Admixture Services (PIVAS),so as to ensure clinical rational use of drugs.METHODS:Statistical and retrospective analysis was conducted on the unreasonable medical orders of anti-tumor drugs interfered by PIVAS pharmacists from 2014 to 2018 in Beijing Cancer Hospital (hereinafter referred to as “our hospital”).RESULTS:From 2014 to 2018,a total of 381 080 medical orders for anti-tumor drugs were received from PIVAS in our hospital,among which 2 014 were unreasonable,and the total irrationality rate was 0.53%.The success rate of intervention by PIVAS pharmacists increased from 99.36%(467/470) in 2014 to 100.00%(379/379) in 2018,and the rate of improper medical orders of anti-tumor drugs decreased from 0.74%(470/63 115) in 2014 to 0.40%(379/93 766) in 2018.Unreasonable medical orders for anti-tumor drugs mainly included 6 types,such as unreasonable dosage of solvent,improper submission of medical orders and unreasonable selection of solvent.CONCLUSIONS:The intervention effects of PIVAS pharmacists against unreasonable medical orders of tumor drugs in our hospital is significant,which can effectively reduce the unreasonable application of drugs.And the effectiveness,safety and economy of the clinical drugs are largely guaranteed.
