摘　　要：目的提高临床药师对肝素致血液透析患者血小板减少症实施有效药学服务的能力。方法临床药师对1例使用肝素抗凝的血液透析患者出现血小板减少症进行药学监护,协助医师进行不良反应处理及治疗药物选择。结果经过替代抗凝治疗,患者血小板回升至接近基础值,未出现血栓栓塞并发症。结论临床药师参与药源性血小板减少症患者的治疗与监护,可减少不良反应的发生,保障患者的用药安全。Objective To improve the ability of clinical pharmacists to implement effective pharmaceutical services for heparin-induced thrombocytopenia in hemodialysis patients. Methods The clinical pharmacist performed pharmacological monitoring on thrombocytopenia in a hemodialysis patient using heparin anticoagulation to assist the physician in adverse reaction treatment and treatment drug selection. Results After replacement of anticoagulant therapy, the patient's platelets rose back to near baseline and no thromboembolic complications occurred. Conclusion The participation of clinical pharmacists in the treatment and monitoring of patients with drug-induced thrombocytopenia can reduce the occurrence of adverse reactions and ensure the safety of patients.
