超早期功能锻炼对老年骨质疏松患者全髋关节置换术后康复的影响

摘　　要：目的探讨超早期功能锻炼对老年骨质疏松患者全髋关节置换术后康复的影响。方法自2014年12月-2017年12月,前瞻性收集南宁市第一人民医院收治的老年骨质疏松患者92例,均行生物型假体全髋关节置换术。术后将患者随机分为观察组和对照组,每组46例。观察组给予超早期功能锻炼,对照组术后给予常规治疗。观察2组患者假体稳定性和功能恢复情况。结果2组患者术后6、24、48h时疼痛视觉模拟评分差异均无统计学意义(P>0.05)。观察组患者住院时间短于对照组(P<0.05)。观察组患者术后腹胀发生率和下肢静脉血栓发生率明显低于对照组(P<0.05)。2组其余不良反应和并发症发生率差异无统计学意义(P>0.05)。术后1个月观察组Harris评分显著高于对照组(P<0.05)。2组患者术后2个月和3个月Harris评分差异无统计学意义(P>0.05)。结论老年骨质疏松患者生物型假体全髋关节置换术后超早期功能锻炼可减少术后并发症,促进髋关节功能快速恢复,且不增加假体下沉距离。Objective To explore the effect of ultra-early functional exercise on the rehabilitation of elderly patients with osteoporosis after total hip arthroplasty.Methods From December 2014 to December 2017,92 elderly patients with osteoporosis admilted to the First Peopled Hospital of Nanning were prospectively collected.All patients underwent total hip replacement with biological prosthesis.The patients were randomly divided into an observation group and a control group,46 cases in each group.The observation group was given ultra-early functional exercise,while the control group was given routine treatment after operation.The prosthesis stability and functional recovery were observed in both groups.Results There was no significant difference in visual analogue scores of pain between the two groups at 6,24 and 48 h after operation(P>0.05).The hospitalization time of the patients in the observation group was shorter than that in the control group(P<0.05).The incidence of abdominal distension and lower limb venous thrombosis in the observation group was significantly lower than that in the control group(P<0.05).There was no significant difference in the incidence of other adverse reactions and complications between the two groups(P>0.05).The Harris score of the observation group was significantly higher than that of the control group at 1 month after operation(P<0.05).There was no significant difference in Harris score between the two groups at 2 and 3 months after operation(P>0.05).Condusion Ultra.early functional exercise after total hip replacement with biological prosthesis in elderly patients with osteoporosis can reduce postoperative complications,promote rapid recovery of hip function,and do not increase the prosthesis sinking distance.
