摘 要:目的探讨敲除鲍曼不动杆菌耐药结节化细胞分化家族(RND)外排泵基因adeB、adeB-like、adeFGH、adeIJK对耐药性的影响。方法 采用自杀性载体pMo130-Telr同源重组法敲除鲍曼不动杆菌AYE的RND外排泵相关基因。琼脂50%稀释法或微量肉汤稀释法对敲除前后菌株进行17种抗菌药物的最低抑菌浓度(MIC)检测。结果 成功获得AYE△adeB、AYE△adeB-like、AYE△adeFGH、AYE△adeIJK菌株。1)AYE△adeB菌株MIC值降低到原MIC的1/4或更低的抗菌药物有:亚胺培南、舒巴坦、氨苄西林舒巴坦、头孢西丁、阿米卡星、卡那霉素、环丙沙星、米诺环素、替加环素;MIC值降低到原MIC的1/2的抗菌药物有:美罗培南、氨苄西林、头孢哌酮、头孢哌酮舒巴坦、头孢他啶、左氧氟沙星、多粘菌素E。2)AYE△adeB-like菌株MIC值降低到原MIC的1/2的抗菌药物有:美罗培南、氨苄西林、头孢西丁、环丙沙星、左氧氟沙星、米诺环素、替加环素、多粘菌素E。3)AYE△adeIJK菌株MIC值降低到原MIC的1/2的抗菌药物有:美罗培南、氨苄西林、左氧氟沙星。4)AYE△adeFGH菌株MIC值降低到原MIC的1/2的抗菌药物只有多粘菌素E。结论 RND外排泵基因对鲍曼不动杆菌耐药性具有不同程度影响,其中adeB对鲍曼不动杆菌AYE的耐药性影响最明显。Objective To investigate the effect of knocking out the resistance-nodulation-cell-division family (RND) efflux pump genes of Acinetobacter baumannii such as adeB, adeB-like, adeFGH and adeIJK on drug resistance. Methods The RND efflux pump genes adeB, adeB-like, adeFGH, and adeIJK in Acinetobacter baumannii AYE strains were knocked out by the homologous recombination method with the suicide vector pMo130-Tel r. The minimum inhibitory concentration (MIC) of 17 Antimicrobial Drugs in the Strains before and after knockout was detected by the agar double dilution method or micro broth dilution method. Results The AYE strains with RND efflux pump knockout such as AYE△ adeB, AYE△ adeB-like, AYE△ adeFGH and AYE△ adeIJK were successfully obtained. When the efflux pump gene adeB was knocked out, the antibacterial drugs with the MIC value reduced to 1/4 or lower were imipenem, sulbactam, ampicillin sulbactam, cefoxitin, amikacin, kanamycin, ciprofloxacin, minocycline and tigecycline, and the antibacterial drugs with the MIC value reduced to 1/2 included meropenem, ampicillin, cefoperazone, cefoperazone sulbactam, ceftazidime, levofloxacin and polymyxin E. When the efflux pump gene adeB-like was knocked out, the antibacterial drugs with the MIC value reduced to 1/2 were meropenem, ampicillin, cefoxitin, ciprofloxacin, levofloxacin, minocycline, tigecycline and polymyxin E. When the efflux pump gene adeIJK was knocked out, the antibacterial drugs whose MIC value reduced to 1/2 were meropenem, ampicillin and levofloxacin. When the efflux pump gene adeIJK was knocked out, the antibacterial drugs with the MIC value reduced to 1/2 were meropenem, ampicillin and levofloxacin. When the efflux pump gene adeFGH was knocked out, the antibacterial drug whose MIC value reduced to 1/2 was only polymyxin E. Conclusion The RND efflux pump gene has different effect on the resistance of Acinetobacter baumannii, and adeB has the most obvious effect on the resistance of Acinetobacter baumannii AYE strains.
