摘 要:目的:探讨智能盆底超声联合多平面成像技术在评估产后盆腔脏器脱垂(pelvic organ prolapse,POP)中的应用价值。方法:选取2016年9月至2018年12月院内90例产后POP产妇作为POP组,同期选取90例健康产妇作为健康组,2组均行静息和Valsalva动作状态下智能盆底超声联合多平面成像技术检查。比较2组静息和Valsalva动作状态下的泌尿裂孔前后径(AP)、左右径(LR)、面积(A),肛提肌厚度(LT)、膀胱尿道后角(PVA)和膀胱颈移动度(BNM)、尿道旋转角(URA),并分析智能盆底超声联合多平面成像技术对产后POP的评估价值。采用SPSS 22.0软件进行统计学分析。结果:POP组静息和Valsalva动作状态下泌尿裂孔AP、LR、A,LT、PVA和BNM、URA明显高于健康组,差异有统计学意义(P<0.05);智能盆底超声联合多平面成像技术评估产后POP的灵敏度、特异度、准确度分别为93.33%、91.11%、92.22%,与实际结果基本一致(P<0.05)。结论:智能盆底超声联合多平面成像技术可有效评估产后产妇泌尿裂孔肛提肌结构和功能的变化,有利于及早有效评估产后POP的情况,值得临床推广。Objective To discuss the application value of intelligent pelvic floor ultrasound combined with multiplanar imaging in evaluating the pelvic organ prolapse (POP). Methods Totally 90 parturients with postpartum POP in some hospital from September 2016 to December 2018 were enrolled into a POP group, and 90 healthy parturients without POP were selected as a health group during the same period. They were given pelvic floor ultrasound combined with multiplanar imaging under resting state and Valsalva motion. The two groups had the indexes at rest and Valsalva motion states compared including the anterior and posterior diameters (AP), left and right diameters (LR), area (A), levator ani muscle thickness (LT), posterior horn of bladder and urethra (PVA), bladder neck mobility (BNM) and urethral rotation angle (URA) of the urinary hiatus. The effect of intelligent pelvic floor ultrasound combined with multiplanar imaging was assessed in evaluating the pelvic organ prolapse. SPSS 22.0 software was used for statistical analysis. Results The POP group had the AP, LR, A, LT, PVA, BNM and URA significantly higher than those of the health group (P<0.05), and the sensitivity, specificity and accuracy by intelligent pelvic floor ultrasound combined with multiplanar imaging were 93.33%, 91.11% and 92.22% respectively, which were almost the same as those by actual measurement (P<0.05). Conclusion Intelligent pelvic floor ultrasound combined with multiplanar imaging effectively evaluates the changes of the structure and function of urogenital hiatus levator ani in postpartum parturients with POP, and it is conducive to assess postpartum POP early and effe-ctively.
