摘　　要：目的探讨和研究不同水平呼气末正压对婴幼儿右颈内静脉横截面积影响的情况。方法选取2018年1月至2019年2月在本院接受治疗的60例插管全身麻醉患儿作为研究对象,并使用不同的呼气末正压值对患儿进行干预,观察对照患儿在0、2、4、6cmH2OPEEP下的右颈内静脉的横截面积。结果使用不同的呼气末正压后,在较高水平PEEP下的右颈内静脉的横截面积也就越大,并且和未进行呼气末正压处理之前的右颈内静脉横截面积相比,使用后的明显更大,差异具有统计学意义(P<0.05)。结论在临床上针对需要进行插管全身麻醉的患儿使用4cmH2OPEEP的干预结果最好,右颈内静脉的横截面积最大并对患儿循环的影响可忽略不计。Objective To investigate and study the effect of different levels of positive end-expiratory pressure on the cross-sectional area of right internal jugular vein in infants. Methods 60 children with intubated general anesthesia who were treated in our hospital from January 2018 to February 2019 were selected as the research object. Different positive end-expiratory pressure values were used to intervene the children. The crosssectional area of right internal jugular vein in the control group was observed under 0, 2, 4, 6 cm H 2 O PEEP. Results After using different positive end-expiratory pressure, the cross-sectional area of the right internal jugular vein was larger at a higher level of PEEP. Compared with the cross-sectional area of the right internal jugular vein before the treatment of positive end-expiratory pressure, the cross-sectional area of the right internal jugular vein after the use was significantly larger (P<0.05). Conclusion In clinic, 4 cm H 2 O PEEP is the best intervention for children who need general anesthesia by intubation. The cross-sectional area of right internal jugular vein is the largest, and the influence on circulation of children can also be neglected.
