摘　　要：目的观察直接前方入路(direct anterior approach,DAA)髋关节置换术治疗高龄股骨颈骨折的临床疗效。方法选取2017年8月至2018年4月收治的高龄股骨颈骨折36例,采用DAA技术行髋关节置换,其中女21例,男15例,年龄71~89岁,平均(79.40±4.80)岁。结果髋关节置换手术切口长(8.12±1.44)cm,手术时间(75.72±13.54)min,术中出血量(165.27±27.19)ml,术后引流量(124.63±16.57)ml,术后下地活动时间(1.51±0.68)d,术后6个月髋关节Harris评分为(92.51±5.83)分,5例出现大腿外侧局部麻木,无脱位病例。结论高龄股骨颈骨折经DAA髋关节置换,软组织损伤小,并发症低,利于术后早期功能锻炼,是一种可靠微创治疗方式。Objective To observe the clinical results of direct anterior approach (DAA) in hip arthroplasty for treatment of femoral neck fracture in the elderly. Methods From August 2017 to April 2018, 36 femoral neck fracture patients underwent hip arthroplasty through DAA, including 21 females and 15 males, aged from 71 years to 89 years with an average of (79.40±4.80) years. Results The length of incision was (8.12±1.44) cm, the operation time was (75.72±13.54) min, the intraoperative bleeding was (165.27±27.19) ml, and the postoperative drainage was (124.63±16.57) ml, the leaving bed time was (1.51±0.68) d, The Harris score was (92.51±5.83), 5 cases suffered local numbness of lateral thigh, no dislocation case. Conclusion Hip arthroplasty through DAA for femoral neck fracture in the elderly has advantages of minimal soft tissue invasion, low complica- tion, facilitate early functional recovery. It is a reliable minimally invasive treatment.
