软坚通脉颗粒对DLAO患者部分炎症因子的影响

摘　　要：目的:探讨软坚通脉颗粒对痰湿证DLAO患者的临床疗效及其部分炎症的影响。方法:选取69例DLAO一期或二期的痰湿证患者,随机分为治疗组和对照组。对照组(n=35)给予标准抗凝、稳定斑块、扩血管等治疗;治疗组(n=34)在对照组基础上加服软坚通脉颗粒,疗程为4周。观察两组治疗前后血清APN、CysC、hs-CRP、IL-6、PCT水平,评价两组治疗前后中医证候评分、临床症状积分的疗效。结果:治疗后治疗组中医证候评分(97%vs51%,P<0.01)及临床症状评分(97%vs80%,P<0.05)有效率均明显高于对照组。治疗组治疗前后血清APN分别为(4.77±0.41)μg/mL和(5.16±0.48)μg/mL;CysC分别为(1.33±0.45)mg/L和(1.02±0.26)mg/L;hs-CRP分别为(6.71±4.31)mg/L和(3.43±2.77)mg/L;IL-6分别为(12.14±8.53)pg/mL和(6.21±4.83)pg/mL;治疗组较对照组的血清APN显著升高(P<0.05),血清CysC、hs-CRP、IL-6显著降低(P<0.05);两组血清PCT在治疗后无统计学差异(P>0.05)。结论:软坚通脉颗粒可上调痰湿证DLAO患者的血清APN水平,抑制血清CysC水平、hs-CRP、IL-6水平,具有一定的抗炎、抗动脉粥样硬化作用。Objective To investigate the clinical effect of Ruanjian Tongmai Granule on DLAO patients with phlegm-dampness syndrome and its inflammatory mechanism.Methods Sixty-nine DLAO patients with phlegm-dampness syndrome were randomly divided into treatment group and control group.The patients in control group(n=35)were given standard anticoagulation,stable plaque,vasodilation and other treatments;the patients in treatment group(n=34)were given Ruanjian Tongmai Granule on the basis of the control group,and the course of treatment was four weeks.The levels of serum APN,CysC,hs-CRP,IL-6 and PCT were observed before and after treatment in two groups.The scores of TCM syndromes and clinical symptoms were evaluated before and after treatment.Results After treatment,the effective rates of TCM syndrome score(97%vs 51%,P<0.01)and clinical symptom score(97%vs 80%,P<0.05)in the treatment group were significantly higher than those in the control group.The serum APN of the treatment group before and after treatment was(4.77±0.41)μg/mL and(5.16±0.48)μg/mL,CysC was(1.33±0.45)mg/L and(1.02±0.26)mg/L,hs-CRP was(6.71±4.31)mg/L and(3.43±2.77)mg/L,and IL-6 was(12.14±8.53)pg/mL and(6.21±4.83)pg/mL.The serum APN levels in the treatment group were significantly higher than those in the control group(P<0.05),the serum CysC,hs-CRP and IL-6 levels in the treatment group were significantly lower than those in the control group(P<0.05),and the serum PCT levels were not significantly different between the two groups(P>0.05).Conclusion Ruanjian Tongmai Granule can up-regulate serum APN levels,inhibit serum CysC,hs-CRP and IL-6 levels in DLAO patients with phlegmdampness syndrome,and has certain anti-inflammatory and anti-atherosclerosis effects.
