当归四逆汤组方在游离皮瓣修复手部组织缺损中的应用

摘　　要：目的:观察当归四逆汤组方在游离皮瓣修复手部组织缺损中的临床疗效。方法:将60例手部组织缺损患者随机分为两组各30例,对照组在游离皮瓣修复术后常规抗感染、抗凝、抗痉挛治疗;观察组除常规治疗外,结合当归四逆汤口服,连用7d。两组的皮瓣的修复情况通过成活率和优良率两个指标进行比较。计量资料通过完全随机两样本t检验或进行比较;计数资料则通过卡方检验或Fisher确切概率法进行比较;等级资料通过Wilcoxon秩和检验进行比较。结果:观察组优14例,良9例,可7例,差0例,优良率77%,成活率100%;对照组优8例,良7例,可14例,差1例,优良率50%,成活率97%。就优良率而言,两组差别有统计学差异,且观察组优于对照组(P<0.05);就成活率而言,两组未见统计学差异(P>0.05)。结论:游离皮瓣修复手部组织缺损术后,应用当归四逆汤可以有效提高皮瓣优良率,获得满意的治疗效果。Objective To observe the clinical curative effect of Danggui Sini Decoction(DSD)on the repair of hand tissue defects in free skin flap.Methods Sixty patients with hand tissue defect were randomly divided into two groups,30 patients in each group,and the patients in control group were treated with traditional antiinflammatory,anticoagulant and anticonvulsive therapy after free skin flap repair.In addition to the traditional treatment,the patients in observation group were given combination with DSD for 7 d after operation.The repair of skin flaps of two groups was compared by survival rate and superior rate.The measurement data were compared by the t-test of two samples.The counting data were compared by chi-square test or Fisher exact probability method.The rank data were compared by Wilcoxon rank sum test.Results In the observation group,there were excellent 14 cases,good 9 cases,medium 7 cases,and poor 0 case,with superior rate of 77%and survival rate of 100%.In the control group,there were excellent 8 cases,good 7 cases,medium 14 cases and poor 1 case,the superior rate was 50%,and the survival rate was 97%.There were statistically significant differences between the two groups in terms of superior rates,and the observation group was superior to the control group(P=0.032).In terms of survival rate,no statistical difference was found between the two groups(P=0.500).Conclusion In free flap in repairing tissue defect postoperative,DSD can effectively enhance the survival rate of flap and obtain satisfactory therapeutic effect.
